0024372886 2017-10-24 Production Wells No Violations The purpose of my inspection was a status check on a production well. The pad is named Quintel and it consists of 10 wells. The well was equipped with a production tree style wellhead. Production valves were in the open position. Production and annular pressure monitoring was present. Gauges were covered with plastic today due to the wellheads being recently painted. The production equipment consisted of ten gas production units, fourteen 400 barrel tanks, three compressors, and four combustors. Identification was present and production records were current. A Myers Well Services truck was on location hauling produced fluids from the tank battery and Antero well tender was on location. The pipeline tie in was located on the West side of the pad. The identification was located at the wellhead and the production records were current.

0357555189 2016-03-31 Casing String Witnessed No Violations I am onsite to witness the surface casing installation and cementing operations. Patterson Drilling installed 1052 of 13.375 J-55 casing after drilling 1058 of 17.5 borehole. Halliburton cemented the casing in place with 1030 sacks of HalCem. I witnessed fifty barrels of cement circulate to surface. Immediately upon landing the latch-down plug, Halliburton increased displacement pressure by 500 psi and held the pressure for five minutes with a less than ten percent decline.
0950060964 2016-04-04 Casing String Witnessed No Violations I am onsite to witness the casing installation and cementing operations. I witnessed the drilling rig, Patterson Drilling 342, land the 9.625 casing in a Camron wellhead at a depth of 7537. The casing was cemented in place by Halliburton. Halliburton utilized 512 barrels of ExtendaCem and 78 barrels of HalCem.

1457524190 2016-03-08 Casing String Witnessed No Violations Status check inspection to witness the conductor casing cement job found Express Energy has drilled a 100 x 32 conductor bore and ran 100 od 26 conductor pipe. Petroset cement service cemented the casing using 330 sacks of class A cement with circulation established and maintained throughout the cement job and 10 barrels of cement returns witnessed to surface inside the cellar. The cement mix weight was verified and the mix water was tested for PH & Chlorides.
