0292019143 2017-07-11 Completion Testing No Violations The purpose of my inspection was a status check on completion operations. During my inspection the well was shut in and equipped with a production tree style wellhead. Multiple contractors are on location installing production lines, fittings, and valves to the wellhead.
0345675942 2016-11-02 Casing String Witnessed No Violations On the day of my inspection I witnessed that Frontier Drilling Rig #32 had drilled a 17.5 inch diameter hole to the depth of 1052 feet. Wyoming Casing Crew ran 1042 feet of 13.375 inch 54.5 pound per foot J-55 casing. Frontier Drilling Rig #32 had to wash down the last joint of casing pumping 80 barrels of fresh water. C & J Energy Services was on site and gained circulation with 102 barrels of fresh water, pumped 25 barrels of gel, 10 barrels of fresh water and cemented casing with 1009 sacks of Class A cement weighing 15.6 pounds per gallon and a yield of 1.19 cubic foot per sack cement. Cement additives included 2% calcium chloride and .25 pound per sack of cello-flake. Cement weight was verified by pressurized mud scales. Cement was displaced with 154 barrels of fresh water and circulated to surface with good returns of 50 barrels. The plug was landed at 450 pounds per square inch and pressured to 960 pounds per square inch for 5 minutes and the floats held with 1bbl back. Mix water was tested revealing chlorides of <31 parts per million, pH 7 and 60 degrees.
0397642094 2018-01-04 Production Wells No Violations The purpose of my inspection was a status check on a production well. The well was equipped with a production tree style wellhead. Production valves were in the open position. The production equipment consisted of twelve 400 barrel tanks within steel line containment, six sand traps, and six gas production units. The pipeline tie in was located on the west side of the pad. The identification was located at the wellhead. Three Antero workers were maintenance production equipment and Myers Trucking were hauling produced fluids from the tank battery.

The sand traps were flowing back into a 400 barrel gas buster tank. The pipeline tie in was located on the west side of the pad. The identification was located at the wellhead. Two Myers trucks were hauling produced fluids from the tank battery during inspection.

Also on location is QES Pressure Control (AKA Great White) providing snubbing unit services. The well head assembly consists of a Cameron sub base and (B) Section main lower well head assembly. Sitting atop the lower section is a (BOP) blow out prevention consist owned by Bass Energy consisting of a manual master valve-
1178807307 2017-06-05 Completion Testing No Violations The purpose of my inspection was a status check. US Well Services and Antero Company men were on location. During my inspection hydraulic fracturing operations were currently shut down and there was not any equipment in operation on the pad.
1288073527 2016-09-09 Casing String Witnessed No Violations I arrived on location to witness Express Energy Services set conductor casing on the WOLF UNIT 2H. Prior to my arrival Inspector Conrad had approved them to set the conductor to 80 feet. We set the conductor using 257 sacks of class A cement with 2% NaCl and about a half a pound of flake. The weight of the cement was 15.6 pounds and had a 1.18 yield with a 32.19 mix. We used a total of 43.35 barrels of fluid for displacement. When we finished setting the casing we had a 10 foot shoe. After we had finished setting the conductor we had about 20 barrels of cement come back to surface and 45 barrels of water come to surface giving us a total of 65 barrels total to surface.

1375036605 2017-06-22 Completion Testing No Violations Status check inspection on the well completion process found a service rig on the hole owned by Fortis Energy. The rig is drilling out all the completion plugs to the well bore total depth inside the 5.5 diameter production casing. Upon completion the rig will pull the drill string keeping the well bore loaded with fluid and static while pulling the string to avoid any well bore gas migration. Upon recovering the 2.875 diameter workover string the production string will be inserted into the well bore.

1496507130 2017-06-03 Completion Testing No Violations Status check inspection on the well completion process found 41 out of 45 stages have been stimulated by US Well Service. All stimulation operations are ceased until further notice from ODNR. The company men on site were notified that nothing was to be done to the well bore until further discussion could take place regarding the stimulation process.

1718903786 2017-03-06 Preliminary Restoration No Violations Preliminary Restoration status check inspection accompanied by ODNR County Inspector Austin Gutridge. The area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is stoned and in good repair. Signage is posted at the well pad entrance. Halliburton crews are on site preparing to begin the well stimulation 5.5 diameter production casing toe preparations.
