0512147343 2016-12-12 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Frontier Drilling Rig #32 was drilling their lateral with an 8.75 inch diameter bit at 12653 feet on 10.5 pound per gallon (ppg) mud simulating 11.8 ppg. Frontier Drilling Rig #32 is planning on drilling to projected total depth of 17200 feet. I completed walk around with Antero Consultant Steve Guffey revealing proper containment was in place and the 5.5 inch production casing was being offloaded.
0574806350 2016-10-26 Casing String Witnessed No Violations On the day of my inspection I witnessed that Frontier Drilling Rig #32 had drilled a 17.5 inch diameter hole to the depth of 1052 feet. Wyoming Casing Crew ran 1042 feet of 13.375 inch 54.5 pound per foot J-55 casing. Frontier Drilling Rig #32 had to wash down the last joint of casing. C & J Energy Services was on site and gained circulation with 182 barrels of fresh water, pumped 25 barrels of gel, 10 barrels of fresh water and cemented casing with 1009 sacks of Class A cement weighing 15.6 pounds per gallon and a yield of 1.19 cubic foot per sack cement. Cement additives included 2% calcium chloride and .25 pound per sack of cello-flake. Cement weight was verified by pressurized mud scales. Cement was displaced with 156 barrels of fresh water and circulated to surface with good returns of 47 barrels. The plug was landed at 420 pounds per square inch and pressured to 940 pounds per square inch for 5 minutes and the floats did not hold. C & J pressured up again to 985 pounds per square inch with the floats not holding again. C & J pressured up for a third time to 1020 pounds per square inch for 5 minutes and the floats held with 1 barrel back to the tanks. Mix water was tested revealing chlorides of <31 parts per million, pH 7 and 60 degrees.

Production valves were in the open position. The production equipment consisted of twelve 400 barrel tanks within steel line containment, six sand traps, and six gas production units. The sand traps were flowing back into a 400 barrel gas buster tank. The pipeline tie in was located on the west side of the pad. The identification was located at the wellhead. Two Myers trucks were hauling produced fluids from the tank battery during inspection.

The production equipment consisted of twelve 400-barrel tanks, six sand traps, and six gas production units. The pipeline tie in was located on the west side of the pad. The identification was located at the wellhead. Myers Trucking was hauling produced fluids from the tank battery.

All stimulation operations are ceased until further notice from ODNR. The company men on site were notified that nothing was to be done to the well bore until further discussion could take place regarding the stimulation process.

Three Antero workers were maintenance production equipment and Myers Trucking were hauling produced fluids from the tank battery.
1545067187 2016-09-10 Casing String Witnessed No Violations I arrived on location to witness Express Energy Services set conductor casing on the WOLF UNIT 1H. Prior to my arrival Inspector Conrad had approved them to set the conductor to 80 feet. We set the conductor using 257 sacks of class A cement with 2% NaCl and about a half a pound of flake. The weight of the cement was 15.6 pounds and had a 1.18 yield with a 32.19 mix. We used a total of 43.35 barrels of fluid for displacement. When we finished setting the casing we had a 10 foot shoe. After we had finished setting the conductor we had about 18 barrels of cement come back to surface and 42 barrels of water come to surface giving us a total of 60 barrels total to surface.
2012233242 2016-12-14 Drill / Deepen / Reopen No Violations I received a call from Antero Consultant Steve Guffey stating that Frontier Drilling Rig #32 reached total depth of their lateral at 17300 feet at 2220 on 12/13/16 and were currently tripping out of the wellbore with 5.5 inch diameter casing to ran and cementing to follow.
2092551203 2017-03-06 Preliminary Restoration No Violations Preliminary Restoration status check inspection accompanied by ODNR County Inspector Austin Gutridge. The area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is stoned and in good repair. Signage is posted at the well pad entrance. Halliburton crews are on site preparing to begin the well stimulation 5.5 diameter production casing toe preparations.
