1133744975 2018-03-25 Completion Testing No Violations Status check inspection found the well has been drilled and stimulated with a Fortis Energy service rig and QVS Company tubing snubbing unit set up on the well drilling out the completion plugs on stage 59 out of 86 total stages completed. The well bore was perforated and stimulated between the depths of 7,764 and 24909. The company is maintaining well control over the 2,800 psi to 3,000 psi using managed pressure completion with 9.7 pound per gallon bring water. The well head is a Cameron Well Head with a (BOP) Blow Out Preventer stack on the well head owned by Fortis Energy, the last BOP test date was 03/22/2018. Falcon Flow Back Service is providing flow back and annular pressure monitoring with all annular pressures checked every 2 hours and (0) Zero annular pressure reported on all the well head annulis. This is the 2nd of 5 wells on the pad to be completed. The 2.375 N-80 production tubing will be set at 7,568.
1234143613 2017-04-20 Casing String Witnessed No Violations The purpose of my inspection was to witness 100 feet of the 26 inch diameter conductor being cemented in a 30 inch diameter hole that is 100 feet in depth. Express Energy Services drilled the conductor with a rotary rig and auger bit. No water was encountered during the drilling process. During my inspection Express Energy Services was on location to complete the cement operations. Job calculations were discussed with Express and Antero Company Man. A pre job safety meeting was held with all personnel on location. I witnessed Express pump fresh water and gel ahead of 330 sacks of class A cement with 2% gel, displaced with 55.74 barrels of fresh water, and cement returns were established. The cellar floor was cemented making the depth of the cellar 6 feet.
1321597069 2017-10-19 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. The pad is named Andes and it consists of 5 wells. During my inspection the lease road was paved with an asphalt surface and the well pad had a gravel surface surrounded by a berm. There were not any erosion issues within the wellsite boundary. Contactors were on location setting up the production equipment. Location has met Preliminary Restoration Requirements listed within 1509.072 (A).
1494076694 2018-05-23 Production Wells No Violations Status check inspection found a total of 5 wells on the well pad. Each well has had the drilling and completion process complete and is in the initial production stages. The wells are being produced through choke manifolds and sand traps with a 3rd party flow back contractor on site monitoring the initial production process. All well bore annulis were monitored through the completion stages and the wells do have flow lines valves and monitoring points on the 13.375 and 9.625 well head sections. There is a total of 5 glycol heaters on site. Also present on the East side of the well pad is a production fluid tank farm consisting of a total of 14 steel tanks of 400 barrels fluid capacity each, the tanks are situated inside a polymer lined steel containment dike area with the well ad perimeter berm serving as the primary containment with the perimeter drain valves closed. Multiple tank trucks were on site hauling both production fluid and water.
1499784753 2017-07-11 Drill / Deepen / Reopen No Violations The purpose of my inspection today was a status check on drilling operations. During my inspection HP Drilling Rig 519 was drilling the 8.5 inch diameter borehole at 8013 feet at 82 degrees within the curved section of the wellbore. The rig was utilizing an 11.2 pound per gallon mud weight while drilling with a balanced hydrostatic pressure method.

During my inspection I witnessed US Well Services setting up on location. The well was equipped with a frac stack and goat head flange style wellhead. I received an email from the Antero stating that they will be finishing rigging up this week.

1672721037 2018-01-02 Completion Testing No Violations The purpose of my inspection today was to witness hydraulic stimulation operations. During my inspection I witnessed United States Well Services stimulate stage 11 with a breakdown pressure of 6636 pounds per square inch, an average treating pressure of 9199 pounds per square inch, an average rate of 68 barrels per minute, and a total of 407991 pounds of sand proppant pumped.
1970747975 2017-06-11 Casing String Witnessed No Violations The purpose of my inspection was to witness 1548 feet of the 13.375 inch diameter intermediate casing being cemented within a 17.5 inch diameter borehole that is 1548 feet in depth. During my inspection I witnessed the last joint of casing being landed, Wyoming Casing Crew rigging down, and Schlumberger Cementing rigging up. A pre-job safety meeting was held with all personnel on location. Schlumberger pumped 30 barrels of gel, 10 barrels of fresh water, and 252 barrels of cement. The cement was Class A cement weighing 15.6 pounds per gallon. I witnessed Schlumberger pull a cement sample and weigh it using pressurized scales to confirm the weight of 15.6 pounds per gallon. The plug was dropped and 233 barrels of fresh water was used for displacement. An estimated 78 barrels of cement returns were witnessed in the half rounds and floats were check to confirm that they have held.
