0157020026 2017-12-11 Completion Testing No Violations The purpose of my inspection was a status check on hydraulic stimulation operations. During my inspection I witnessed US Well Services setting up on location. The well was equipped with a frac stack and goat head flange style wellhead. I received an email from the Antero stating that they will be finishing rigging up this week.
0557115048 2018-05-23 Production Wells No Violations Status check inspection found a total of 5 wells on the well pad. Each well has had the drilling and completion process complete and is in the initial production stages. The wells are being produced through choke manifolds and sand traps with a 3rd party flow back contractor on site monitoring the initial production process. All well bore annulis were monitored through the completion stages and the wells do have flow lines valves and monitoring points on the 13.375 and 9.625 well head sections. There is a total of 5 glycol heaters on site. Also present on the East side of the well pad is a production fluid tank farm consisting of a total of 14 steel tanks of 400 barrels fluid capacity each, the tanks are situated inside a polymer lined steel containment dike area with the well ad perimeter berm serving as the primary containment with the perimeter drain valves closed. Multiple tank trucks were on site hauling both production fluid and water.
0620561941 2018-03-25 Completion Testing No Violations Status check inspection found the well drilling and stimulation process now complete. A Fortis Energy service rig is on location drilling out the completion plugs and running the production tubing on all 5 wells on the pad with a tubing snubbing unit owned by QVS rigged up on the well head Blow Out Preventer. Well control over the 3,000 psi 5.5 diameter casing internal pressure will be managed with 9.7 to 10.7 pound per gallon brine water with the company utilizing pressure managed drilling to maintain surface control. The well bore was stimulated in 85 stages between the measured depths of 7,978 and 24,816. Falcon Flow Back Service is controlling well pressure at surface and monitoring all annular pressures every 2 hours. The well head is a Cameron Well Head.
0682116884 2017-04-20 Casing String Witnessed No Violations The purpose of my inspection was to witness 100 feet of the 26 inch diameter conductor being cemented in a 30 inch diameter hole that is 100 feet in depth. Express Energy Services drilled the conductor with a rotary rig and auger bit. No water was encountered during the drilling process. During my inspection Express Energy Services was on location to complete the cement operations. Job calculations were discussed with Express and Antero Company Man. A pre job safety meeting was held with all personnel on location. I witnessed Express pump fresh water and gel ahead of 330 sacks of class A cement with 2% gel, displaced with 55.74 barrels of fresh water, and cement returns were established. The cellar floor was cemented making the depth of the cellar 6 feet.
1023363592 2017-10-19 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. The pad is named Andes and it consists of 5 wells. During my inspection the lease road was paved with an asphalt surface and the well pad had a gravel surface surrounded by a berm. There were not any erosion issues within the wellsite boundary. Contactors were on location setting up the production equipment. Location has met Preliminary Restoration Requirements listed within 1509.072 (A).

1494852241 2017-05-13 Casing String Witnessed No Violations The purpose of my inspection today was to witness the cementing of the Surface casing. Schlumberger was on location to cement 394 feet of the 18.625 inch diameter casing within a 24 inch diameter borehole that was 404 feet in depth. A pre-job safety meeting was held with all personnel on location. Schlumberger pumped 225 barrels of fresh water, 30 barrels of gel spacer, 10 barrels water, and 130 barrels of cement at 15.6 pound per gallon. Cement sample was pulled and weighed in pressurized scales confirming a weight of 15.6 pound per gallon. Top plug was dropped and displaced with 110.3 barrels of water. An estimated 50 barrel of cement returns were witnessed in the cellar. The plug was bumped and held for 15 minutes and the floats held.
1495112092 2017-05-17 Drill / Deepen / Reopen No Violations The purpose of my inspection was to witness the formation Integrity testing from 1600 feet to 1615 feet just below the 13.375 inch diameter intermediate casing. H&P Flex Rig 519 resembled the same pressure on the formation as 13 pound mud by using 8.5 pound per gallon fluid and the hydrostatic pressure. During my inspection I witnessed the H&P driller fill the hole with 8.5 pound per gallon fluid, close the pipe rams, circulate fluid through the choke line to remove air, and then begin to stage pressure up slowly until they reach 375 pound per square inch. 375 pound per square inch was held for 15 minutes. The test was successful.
1503503194 2017-08-22 Drill / Deepen / Reopen No Violations The purpose of my inspection was a status check. During my inspection H&P Flex Rig 519 was tripping the drill pipe, bottom hole assembly, and 8.5 inch diameter bit in hole at 5600 feet. The current hole depth is 8834 feet within the lateral section where the rig experienced signal problems with the bottom hole assembly tools and had to trip out to maintenance the lateral assembly.
1504695368 2017-09-05 Drill / Deepen / Reopen No Violations During my inspection HP Drilling Rig 519 was preparing for a rig move after cementing the 5.5 inch diameter production casing. Schlumberger cemented 25042 feet of 5.5 inch diameter casing in a 8.5 inch diameter borehole that was 25055 feet measured depth.
