0411032687 2014-01-10 Orphan No Violations At the time of the inspection the well was leaking trace amounts of oil and gas from the top of the casing head where the tubing has broken. Approximately 1/4 gallon of oil has pooled around the wellhead since the initial cleanup done on 12-27-2013. There was no sign of oil in the water coming from the main tile outlet that drains the field in which the well is located. The well will continue to be monitored by the landowner and the inspector.

The fluid level inside the well was still at the top of the casing head but no significant leaking has occurred since the last inspection on 1-15-2014. This well and the other four wells on this property are being referred to the Orphan Well Program.
1336906263 2013-12-27 Orphan No Violations Inspection done in accordance with Complaint #4569. A temporary earthen dike apprximatly 10 in diamater was built around the casing head and litter was used to soak up any pools of oil. Some contaminated soil was placed in garbage bags and left on location. No further leaking has occurred and the well and tile will be monitored by the landowner or the inspector untill further notice.
1388760622 2013-12-26 Orphan No Violations Inspection done in response to Complaint #4569. Robert Bench called at 1:25pm on 12-26-2013 because of possible oil leaking from a well. At the time of the inspection the well has 8 drive casing to the bedrock (~50) and 6 casing through the fresh water Big Lime (~400). There is a casing head on the 6 casing with two, 2 outlets. The outlet to the west has a 2 plug, but the one to the east has a an elbow and nipple that is open. There is a string of tubing sitting on a casing ring that has broke off right below the collar holding up the string. The rods are still above the casing head still inside the tubing. Oil and water has now filled the wellbore up inside the casing head to just below the casing ring. Approximatly 5 gal of oil have escaped from the well and pooled around the wellhead but the well was not leaking. The field tile around the well all drains out a main into a low area that leads to Sugar Creek ~500 to the north. No oil was evident coming from the main tile outlet.

1567691961 2014-05-20 Orphan No Violations This well started leaking small amounts of oil in December of 2013 and was referred to the Orphan Well Program. At the time of the inspection the fluid level inside the well was 8 below the top of the casing head and is not currently leaking. The small earthen dike and the three garbage bags of contaninated soil have deteriorated. The soil was piled next to the well and the dike was repaired. This dike will only hold a few gallons of fluid so the well needs serviced or plugged as soon as possible.
