0013679111 2002-04-29 Production Wells No Violations See Case # 02-010. 4-29-2002 - Received a complaint from Mike Griffiths, Portage Co. EMA (330-297-3607) regarding a tank battery dike leak. Inspectors Claus and Worstall inspected the site and found evidence of residual crude oil floating on the water within the tank battery dike and residual crude oil leaching out from a graded soilpile located adjacent to the dike. Two absorbent booms were observed in the narrow creek located downgradient from the tank battery dike. The booms had been placed by Mark Griffiths of the Portage Co. EMA. No evidence of contamination was observed in the creek. Inspectors Claus and Worstall also met with Patti Biles, the resident at 2191 St. Rt. 14, who called in the complaint to the Portage Co. EMA. Further observation at the site revealed that the tank battery dike had filled with water and flooded the inground concrete brine vault, causing residual crude oil from the brine vault to float out onto the surface of the water resulting in the contamination at the site. Tim Altier of Great Lakes Energy (1-800-448-8037) was informed of the situation and recommendations were given to correct the current and future problems at this site. Follow-up inspections will be conducted to observe the progress.

During a conversation with Tim Altier, Great Lakes has agreed to remove the concrete brine vault and replace it with an aboveground tank. Inspection of the leachate that was observed down gradient from the tank battery does not appear to be crude and smells like cow manure.

0531914493 2018-04-20 Production Wells No Violations Status check inspection of this Clinton Sandstone well. My inspection found the wellhead equipped with 2.375 tubing and rods and operating with the aid of an electric powered pumping unit. The wellhead and unit are fenced and the man door is locked. This well produces to a diked, fenced, and locked tank battery along the lease road containing two (2) 210 barrel tanks and a vertical separator. Gas production is being recorded 0.2 miles to the west at a meter shack (MOA) along the pipeline right-of-way. The required ID is posted at the wellhead and at the tank battery. No violations.
