This battery is shared with the Reynolds # 1 (PN#21784). My inspection today found that the wells have not been placed back in production and the separator at the tank battery remains disconnected. The flow line valves are closed and the required ID is posted on the tank. I inspected the west end of the impacted area along Headwaters Trail and found the area completely remediated. Grass is well established and the ground is stable. This Compliance Notice will remain open until the requirements in the notice are achieved. |

My inspection found the wellhead equipped with a natural gas-powered pumping unit. A bull plug on the 2 flow line plumbed to the 4.5 casing has a minor gas leak and I will speak with Equity about tightening the plug. This well produces to a tank battery 1000 to the southeast and is shared with the Reynolds #1. The tank battery is diked and contains two (2) 100 barrel tanks and a horizontal separator. 1462377132 | 2016-04-25 | Production Wells | Violations Noted | I was notified by Equity Oil and Gas at 1:38pm on 4/24/2016 of an oil release that occurred on a line from this well and the Reynolds #1 (PN#21784). Inspector Stephen Tompkins responded and reported later that it was not determined if the leak occurred from a flow line or sales line tied to these wells. He reported that 5-8 barrels of oil had been released at two (2) locations west of the tank battery and along the north side Headwaters Trail. I was on location this morning with Kurt Kollar (OEPA), Craig Alderman (Portage Park District), Scott Miller Contracting (SMC), Northeast Oilfield Services, and Equity personnel to begin remediating the spill. A large hole (5x15), 400 southwest of the tank battery (Leak #2, GPS NAD83: 41.28038, -81.10697) contained 5 barrels of crude oil and prevented the spill from migrating downslope through the woods. Northeast vacuumed the oil and water from the hole and SMC excavated 3 down to expose a 2, steel line. Three (3) holes were found in the pipe and clamps were installed to prevent further discharge. The contaminated soil was stockpiled on the west side of the battery in a diked and lined containment area. A second hole (Leak #1, GPS NAD83: 41.28070, -81.10622) was excavated 175 southwest of the tank battery where oil had surfaced in an agricultural field. The line was exposed and clamps were placed on the steel line to prevent further contamination. SMC removed the affected soil to containment and scraped a 120 X 6 path down to clay (10-12 deep) where oil had run across the surface of the ground. The affected topsoil was placed in the lined containment area for removal. Leak #3 occurred 0.5 miles west of the tank battery along the edge of Headwaters Trail. SMC began excavating affected material and hauling it to the containment area. A trench 50L x 8W x 6H was excavated from the ditch back into the old railroad base of stone and the free oil began seeping out into the ditch. Several sumps were constructed to co |

My inspection today found that all of the affected and excavated areas have been restored and the seed bed is well established along the trail. The violations for pollution and contamination and inadequate pipeline strength have been corrected and the old sales line has been permanently abandoned. The violations outlined in the original compliance notice for general safety will remain in effect until that time when they are corrected and prior to placing the Reynolds #s 1 & 2 wells back in production. |

Scott Miller Contracting (SMC) and Northeast Oilfield Services were on location to continue with clean-up operations. SMC was working to fill the large excavation at Leak #3 with stone and clay. The 1400 section of sales line between Leak #2 and the trench on the north side of Headwaters Trail was flushed from west to east with hot water. Northeast was hooked up to the east end of the line and vacuumed all the discharge into the water truck for disposal. The line was flushed until the water ran clear. Northeast placed suction on each end of the line following the hot water treatment to evacuate the line. SMC personnel walked the line during the job and no other release points were found. This process was repeated from east to west between the tank battery and Leak #2 and the lines were evacuated. The open ends of both sections of sales line were threaded and capped and the sales line riser an the tank battery was fitted with a bull plug. SMC continued to haul the remaining affected material from the containment area. I had to leave location and I asked Terry Whitehair to oversee the removal of affected material inside the tank battery. I spoke with Terry later and he told me they moved the separator and removed all the affected material from inside the tank battery. I will continue to monitor the location until reclamation is complete and all affected parties are satisfied. |

Overnight rains had flushed free oil into the containment ditch and Northeast Oilfield Services vacuumed up the free product from the ditch. Scott Miller Contracting (SMC) continued to excavate back into the railroad bed. The sales line and the location of Leak #3 was found (GPS NAD83: 41.27699, -81.11413) seven (7) feet south of the containment ditch, buried in clay, and four (4) feet below grade. Oil continued to seep from the line and I had Equity disconnect and remove the sales line valve at the tank battery and fit the line with a bull plug. This would ensure that no more product could enter the line accidentally. SMC continued to excavate the rock, clay, and soil that was affected by the release down to the east end of the ditch. All the contaminated material was staged at the lined containment area west of the tank battery. W. Pol Contracting was hired to remove the material and it was being trucked to the Kimble Landfill in Dover, Ohio for disposal. All of the stone, soil, and clay will need to replaced after the spill is remediated. A good stopping point was reached and SMC installed orange safety fence along the excavated area fro safety purposes. Clean-up operations will continue tomorrow. |

This morning, Scott Miller Contracting (SMC) hooked up to the west end of the sales line (GPS NAD83: 41.27691, -81.11548) where it was capped and flushed the line with hot water from the steam truck. This section of line (400+ long) was flushed from west to east to the location of Leak #3 on the north side of Headwater Trail. Northeast Oilfield Services captured the flushed oil and water with a vacuum truck hooked on the end of the line. The line was flushed until the water was free of oil. Northeast then placed suction on the west end of the sales line to evacuate any fluids. SMC excavated a trench along the north side of the trail (GPS NAD83: 41.27813, -81.11131) to expose the sales line 880 to the east of Leak #3. The flushing process was repeated east to west, the line was evacuated, and caps were placed on the ends of both sections of the line. Excavation at Leak #3 needed to move forward and a decision needed to be made about a triple-trunk, black walnut tree (15OD) that would require removal if all of the contaminated material was to be removed. A large root of the tree had already been badly damaged during excavation operations and would likely kill the tree. Myself, Terry Whitehair (DOGRM), and Scott Miller consulted with the Park Operations Manager, Craig Alderman, as to what the park desired to have done with the tree. A decision was made to remove the tree, and it was excavated and cut. The remainder of the contaminated material was excavated and removed from the location for disposal. Operations were shut down for the day and the orange safety fencing was placed back around the excavations. An estimated 200 tons of material (rock, clay, and soil) have been removed from the three locations impacted. |
