1521721894 2018-03-21 UrbanProduction Wells No Violations I received a call from Joe Teppert (Enervest) at 1:40pm to inform me that they had a three (3) barrel (126 gallon) oil release at this wellhead. I instructed Joe to report the incident to the Ohio One Call line due to exceeding the reporting limit of more than 25 gallons outside of secondary containment in an urbanized area. I arrived on location at 2:38pm and Enervest personnel and Scott Miller Contracting (SMC) were on location and using the Enervest water truck to vacuum up the free oil. The impacted area was 30 X 20 and most of the crude oil was in a pocket 10 in front of the wellhead. I tested some standing water on the west side of the impacted area with three (3) high range Quantabs and found that the water exceeded the detection limit for chlorides. I instructed the personnel to vacuum up all of the standing water for disposal. The area was then scraped by SMC and all of the impacted material was loaded into a roll-off box. 12-15 tons of material was removed and SMC is going to dispose of the material at the Kimball Landfill. I asked Enervest personnel to treat the impacted area with lime and they will accomplish that task tomorrow (3/22/18). Any residual oil that may surface will be collected with absorbent pads and disposed of properly. It is likely that the source of the leak was the stuffing box and the well will remain shut-in until wellhead is repaired and tested. SMC built a secondary containment dike around the wellhead to help mitigate and contain any future releases. I will monitor the location for any further remedial issues.
1521823812 2018-03-23 Production Wells No Violations Follow-up to an oil release that occurred on 3/21/18. I was on location with Enervest personnel and the faulty two (2) inch valve was removed and replaced with a new Balon ball valve. The wellhead was plumbed back together and the well was tested for 15 minutes with the pumping unit running. No leaks were observed during the test and the well can be placed back into production. The entire impacted area has been coated with lime for remedial purposes and there was no free oil observed on the ground. This issue has been resolved. No violations.
1597922129 2018-03-21 UrbanProduction Wells No Violations I conducted a status check inspection after responding to an oil release at this wellhead. My inspection found the wellhead equipped with 2.375 rods and tubing and operating with the aid of a natural gas powered pumping unit. The well is currently shut in due to a leak found at the wellhead. This well produces to a diked tank battery on location and contains one (1) 140 barrel tank, a 50 barrel poly tank, and a vertical separator. The required identification is posted at the tank battery. No violations.
