1530030721 2018-06-26 UrbanProduction Wells No Violations Status check inspection of this Clinton Sandstone well. My inspection found the wellhead fenced and equipped with 2.375 rods and tubing and a natural gas powered pumping unit. The flow line valves on the tubing and 4.5 production casing are currently closed and the well is shut-in. The well produces through a fenced, horizontal separator north of the wellhead and the chart was last changed on 5/8/18. There is a domestic tap on the separator sales line and the supply valve is closed. The tank battery is diked and fenced and contains one (1) 100 barrel tank. Production records show that the well is not in violation of ORC 1509.62. The required ID is posted at the wellhead and at the tank battery. No violations.
1549884700 2015-09-25 UrbanProduction Wells No Violations I received a call from Joe Teppirt (Enervest) at 9:59am on 9/24/2015 to inform me of an oil release in the tank battery of this well. He told me the pumping unit did not properly shut-off overnight and 2-3 barrels of oil over flowed the tank lid. He said all the oil is in the containment dike and they are sucking it out with a vacuum truck. He told me Scott Miller Contracting was on the way to location to steam out the gravel and the bottom of the tank battery. My inspection today found the tank battery clean and free of oil (I spoke with Scott Miller earlier in the day and he told me that they had dug a sump and steamed out the tank battery with 120 barrels of freshwater). The tank battery is diked and fenced and contains one (1) 100 barrel tank with the proper ID posted on the tank. There is an old, 35 barrel poly tank sitting outside containment and is not being used. I will ask Enervest to remove the old tank from location. The wellhead is fenced and equipped with a natural gas powered pumping unit with 100psi on the 4.5 production casing. The proper ID is posted at the wellhead. The well produces through a heated separator to the east that is fenced and diked. The heating element is not being used at this time and the set-back distance from the wellhead and tank battery meets the requirements outlined in OAC 1501:9-9-05(A)(3). I will continue to monitor the tank battery for any residual oil issues. I found no violations at this time.
