0979733585 2018-06-06 Production Wells No Violations Status check inspection of this Clinton Sandstone well. My inspection found the wellhead equipped with 2.375 rods and tubing and operating with the aid of an electric powered pumping unit. Gauges at the wellhead indicate a pressure of 70psi on the tubing and 65psi on the 4.5 production casing. The unit and wellhead are fenced diked and the man door is off (not required). It is apparent that the stuffing box is leaking as the wellhead and stuffing box are covered in crude oil. There are several quarts of fresh oil on the rock around the wellhead and on a soaked absorbent pad. The leaked oil has stained the rock three (3) feet in front of the wellhead and 8 to the east to the base of the secondary containment wall. The support beams on the front of the unit have snapped in half and likely unleveled the polished rod causing wear on the rubbers in the stuffing box. I called Matt Posthalwait (Supervisor) and left him a message that the well needs to be turned off until the area is cleaned up, the stuffing box is repaired, and the pumping unit is leveled and reset. The switch on the electric power box is currently in the on position. This is not a reportable release (2-3 gallons) and all oil is within the secondary containment area. Matt returned my call and they are going to shut the well off, clean up around the wellhead, and make the necessary repairs. A violation has not been issued at this time.
1513168455 2018-07-31 Production Wells No Violations Follow-up to an inspection conducted on 6/6/18 that found the stuffing box leaking caused by snapped support beams that had unleveled the pumping unit. My inspection today found the support beams have been replaced, the unit has been leveled, the stuffing box has been repaired, and the wellhead has been cleaned off. The stained rock remains in front of the wellhead, is inside secondary containment, and is not mobile. This issue has been resolved. No violations.
