The samples were sent to the Cambridge laboratory for analysis.

The pump was running when we arrived on location and a we witnessed a Stallion employee turn down the RPMs on the injection pump before we could inspect the injection pressure at the wellhead. A gauge on the 5.5 injection tubing indicated an injection pressure of 670psi. A gauge on the annulus between the 8.625 casing and injection tubing indicated a pressure of 300psi. I found no violations at this time.

Set the R4 Halliburton packer in 20,000# tension over the string weight. Universal pressured up the annulus to 900psi. Could not get any higher. Leaks occurred around the wellhead. Shut down. This procedure was redone 4 more times. Could not get pressure higher than 1000psi. Then pressure would bleed down to 840psi and hold. Reset packer 3 more times. From 22,000# tension, 25,000# tension and 27,000# tension. Universal pumped up to 1200 psi. The R4 packer sheered. Will trip out of the hole with the 5 1/2 casing tomorrow. There were no violations at this time.

1361805832 2013-02-23 Salt Water Injection Well No Violations I was on location with Inspector John Fleming this morning to conduct operator and brine hauler compliance inspections at the request of the Uniontown Field Office. The well was staffed by Grover (Stallion) and we informed him of our purpose for being at the well site. He gave us a tour of the facility and explained the operations of the injection well. The well was not actively injecting at the time of our inspection. The idle pressure on the well was monitored from the pump house and indicated 575 psi on the well. The automatic shut-off on the gauge was well below the maximum allowable pressure of 945 psi. The site was clean and there were no leaks observed on any of the equipment. A Stallion 140 barrel tanker (Ohio license plate #TPT6340) arrived at 10:23am to dispose of a load of water. The UIC number (#386) was properly posted on the side of the tanker and is legally registered to haul brine in the State of Ohio. A check of the manifest indicated that the water being hauled was from Eureka Resources Recycling Facility in Lycoming County, Pennsylvania. The tanker was carrying 113.66 barrels of 9.9 lbs/gallon recycled water and unloaded at the facility. There were no violations at this time.

The well was staffed by Grover ( Stallion) and we informed him of our purpose for benign at the well site. He gave us a tour of the facility and explained the operations of the injection well. The well was not injecting at that time. The idle pressure of the well was 575 psi. The site was clean there were no leaks observed on any of the equipment. A Stallion 140 barrel brine tanker (Ohio license plate #PTP6340) arrive on location at 10:23 am. to dispose of a load of water. The UIC number (#386) was posted on the side of the tanker and is legally registered to haul in the state of Ohio. The manifest indicated that the water was from Eureka Resources Recycling Facility in Locoming County, Pennsylvania. The tanker was carrying 133.66 barrels of 9.9lbs/gallon water and unloaded at the facility. There is no violations at this time.

Universal pumped 145 barrels of inhibitor down the casing. The Baker Lok Set packer was set in 20,000# compression at 4030. The annulus was pressured to 1218psi. It lost 8psi in 15 minutes. This well passes UIC test.

The proposed affected area is located due south of the existing tank battery and pump house. The well stake could not be located. West of this area is a likely jurisdictional wetland. Even if the proposed well is out of the wetland, there may be issues with a shallow water table when trying to construct earthen pits to drill the well. The hearest homes are located along SR 183 and are over 500 west of the well.

There were no violations at this time.

Several fittings on the casing were lose, causing the test to fail. A company representative was instructed to tighten the fittings and install a working valve and gauge on the casing.

The BOP was tested to 550psi and lost 5psi in 15 minutes. I did not witness the BOP test. The drilling depth was 619 and drilling with a 12 1/4 bit on air. This inspection satisfies 1501:9-9-03. There were no violations at this time.
