0090513581 2014-03-17 Production Wells No Violations Status check inspection of this Clinton well. The wellhead is equipped with a gas powered pumping unit. The tank battery is diked and contains 1-210 barrel and 1-100 barrel and the proper ID is posted on the tank. A fire-heated, horizontal separator is located to the east of the tank battery and south of the wellhead at a distance that meets the minimum spacing requirements. No violations.
0260827757 2016-02-25 Production Wells No Violations I conducted a status check inspection of this Clinton Sandstone well. The wellhead is equipped and operating with the aid of a natural gas powered pumping unit. The pumping unit is equipped with a 13 horsepower Honda motor. The natural gas supply to the motor is shut-off and the Logic controls are turned off. The well is currently producing and is being operated manually. A gauge on the tubing indicates a pressure of 300psi. The well produces through a heated, horizontal separator and a gauge at the separator indicates a production pressure of 300psi. A new, 7-day metering chart was changed out today (2/25/16) and is recording production. The tank battery is diked and contains one (1) 210 barrel tank and one (1) 100 barrel tank with the proper ID posted on the 210 barrel tank. The lease road and apron are in stable condition and the tubular gate at the entrance is open. The well is tied in and producing to an Orwell Natural Gas rotary metering station on the south side of the lease road entrance. The station is fenced and locked. No violations.

1133283211 2005-11-26 Drill / Deepen / Reopen Violations Noted At 22:15, I received a call from the Aurora Fire Department (Ron at 330-562-7171) reporting numerous gas odor complaints in the vicinity of Winchell, Town Line and Wisperwood Roads. Several attempts by Fire Department personnel to locate the source were unsuccessful. Dominion East Ohio had also investigated and determined that their system was not the source. Capstar Drilling Rig #1 was on the Fugman #13 well for Great Plains Energy in Mantua Township near the intersection between Winchell and Chamberlain Roads, approximately one mile due east of the complaint area. The wind Saturday night was blowing from east to west. I contacted Tim Maple, the toolpusher, and was told that the rig had drilled through the Newburg earlier in the evening on air and had encountered gas. At 22:30, I ordered Capstar to switch to fluid. When I arrived on site at midnight, the switch was nearly complete and the gas odor was diminished. According to notes on the geolograph record, the first gas was encountered at 15:00 at a depth of 3575. Additional gas and water were encountered at 17:30 at 3638. The switch from air to fluid should have occurred much earlier in the evening. Both Capstar and Great Plains should also have notified me of the problem immediately.
