0606793831 2014-12-04 Casing String Witnessed No Violations I was on location to witness the cementing of the 11.75 conductor casing. Poulson Drilling Rig #1 encountered bedrock at 54 and drilled the 14.75 hole to a depth of 92. They ran 2 joints of 32lb/ft. casing to 82 with an 8 landing joint. Universal rigged up, established circulation, and cemented the casing with 15 barrels of Type I cement slurry. 2 barrels of cement circulated and held at the surface. 1179175962 2014-12-05 Casing String Witnessed No Violations I was on location to witness the cementing of the 8.625 surface casing. Poulson Drilling Rig #1 drilled an 11 hole to a depth of 392. Drill cuttings caught by the rig crew indicated the base of the Berea Sandstone right around 300 and Poulson set 359 of surface casing in the hole with an 8 landing joint. Universal rigged up, established circulation, and cemented the casing with 30 barrels of Type I cement slurry. 8 barrels of cement circulated and held at the surface. 1482873846 2014-12-30 Completion Testing No Violations I was on location to inspect completion operations being conducted on this recently drilled Clinton Sandstone well. Universal Well Services had their trucks spotted and was rigged up to the wellhead. I was in the control trailer with Tom Wood (consultant) and Tim Cosma (Cedar Valley). Universal opened the frac valve on the 4.5 production casing and the annular valve on the 8.625 casing and loaded the hole. They began pumping and broke the formation down at 1443psi. I left location to finish plugging operations at PN# 21143. No violations.
1668929350 2015-04-01 Preliminary Restoration No Violations I inspected this location to check the status of preliminary restoration. The drilling pits were solidified and closed in mid-December and the well was hydraulically fractured on 12/30/15. The long winter has delayed restoration operations and the entire location needs to graded and restored. The wellhead is equipped with a pumping unit, but it is not yet in service. The tank battery is partially constructed and contains a horizontal separator, 1-140 barrel tank, 1-100 barrel tank, and a 70 barrel steel tank. None of the equipment is currently plumbed into the separator or other tanks. Preliminary restoration is due in June. No violations.
1817520350 2014-12-09 Casing String Witnessed No Violations I was on location to witness the cementing of the 4.5 production casing. Poulson Drilling drilled a 7.875 hole to a total depth of 4,259 and ran used 4.5 casing to a depth of 4,239 with a float shoe on bottom and a latch-down collar at 4,229. (I have paperwork verifying that the pipe has been drifted and pressure tested). Universal rigged up to the cement head containing a latch-down plug and equipped with a wire tattle-tail. Universal pumped 42 barrels of spacer and then mixed and pumped 57 barrels of Unitropic cement slurry. They dropped the plug and the wire indicated the plug had left the cement head. They displaced the plug to the collar with 67 barrels of water. The plug landed at 915psi and the pressure was increased and held at 1620psi for five minutes with no leak-off. The pressure was released and the floats held. The estimated top of the cement is 2809. See casing ticket for more details on this job. No violations.
1969625006 2015-06-26 Preliminary Restoration No Violations I was on location to check the status of preliminary restoration. Drilling was completed on 12/9/15 and the pits were solidified and closed the next week. The well was completed on 12/30/15 and preliminary restoration is due. My inspection found the site graded and planted with corn. The well was operating with the aid of an electric pumping unit. The valve on the annulus between the 8.625 surface casing and 4.5 production casing was plumbed to surface and open. A gauge indicates 300psi on the casing and there is a domestic hook up at the wellhead. The well produces to a diked tank battery into a horizontal separator operating at 70psi. The tank battery contains one (1) 140 barrel tank, one (1) 100 barrel tank, and one (1) 70 barrel tank. The proper ID is posted at the wellhead and the tank battery. Preliminary restoration is approved. No violations.
