0994293431 2012-12-03 Production Wells No Violations No well identification sign on wellhead. Monitored for gas leak. No noticeable odor. Honeywell PhD6 gas meter detected 0.05 CH4 gas reading units and 5 % of the LEL. Construction of the well is as follows: There is a concrete pad on which a 8-7/8 inch steel casing rises up about 7-inches above the pad. Inside this casing is a 2-inch production casing which continues vertically up about 4-feet above the pad. Where this tubing rises out of the surface casing there is no cement in the annular space. A top hat, traffic cone, was placed above this annular space to keep rain out of the space. Because there is no cement in this annular space at the surface the production tubing is unstable. A brace is attached from the surface casing to near the top of the production tubing to support it. At the top of this tubing is a 2-inch t-fitting. Continuing vertically up there is a 3/4-inch tubing attached with a pressure gauge on top and at the other end of this small diameter tubing. Pressure gauge is reading 57 psi. From the stem of the t-fitting, a 1-3/4 inch tubing traverses horizontally. A shut off valve was installed on this tubing and the valve was in the open position. Continuing from this valve the 1-3/4 inch tubing ends in another t-fitting. Another 1-3/4 inch tubing drops down from this fitting to another t-fitting of the same size. From the stem of this T-fitting there is another shorter 1-3/4 inch tubing traversing and ending with a 1-inch plastic tubing inserted into the tubing with tape wrapped around the connection holding the plastic tube. The plastic tubing is then buried in the ground next to the concrete pad heading in a southerly direction. According to the Ohio Fuels Map there should be another gas well that had no permit number shown on the map west of this location and within 500 feet. Search for this well uncovered no wellhead. I also checked east of this well just in case I had discovered this other well. The RBDMS GPS coordinates for
