| 1160495299 | 2006-10-03 | Plug / Plug Back | No Violations | -On site for Huwer Drilling to set the bottom plug in the well. Well had some oil in it that Bob had pumped (about 4 bbls) that was vacuumed the day before along with salt water. Bob had placed 1430 feet of pipe down the hole for the cement and about 30 feet off the bottom. 50 sacks of Class A cement was placed in the bottom plug. Cement from Ohio Ready-Mix. Bob indicated that when he measured the top of the cement, it was at around 1181 below the surface. The plug was about 280 feet thick. Huwer was planning to set the middle and upper plug on Wednesday, October 4. Bob and Aaron Huber were on the site. Digital photos taken. Huwer was planning to pull the 7 pipe and leave the 8 pipe down to the 115 foot level. Bob also to remove the 100 barrel tank on site. |
| 1160496394 | 2006-10-04 | Plug / Plug Back | No Violations | -On site for Huwer Drilling to set the middle and upper plugs on the well. Huwer set the bottom plug the previous day on 10/3/06. Ohio Ready-mix brought out 50 sacks of class A cement for the middle plug. The diameter of the hole for the middle plug was 6.5 inches. The 50 sacks correspond to 256 feet of cement. Middle plug was placed from around 513 feet to 313 feet below the surface to seal off a freshwater zone. The upper plug included 60 sacks of cement in a 8 diameter hole. The amount of cement should be about 200 feet. When that cement was pumped, it did not circulate all the way to the top. Bob Huwer was to take some equipment for the site before rains hit then come back after the top plug had set and either top it off himself or call Ohio Ready-Mix for more cement. The 8 pipe and conductor pipe would be cut off 30 below the surface, a plate with the well # on it placed, and the area covered and reseeded. Bob was to remove the 100 bbl tank. I did not witness the final topping off and cutting off the 8 casing and conductor pipe. Bob Huwer left a message on 10/10/06 regarding the final plugging of the well. |
| 1164208214 | 2006-11-21 | Final Restoration | No Violations | -On site for final restoration inspection. Bottom plug in well was set on 10/3/06. The remaining two plugs were set on 10/4/06 by Huwer Drilling. Huwer took down the fence around the former well location and tank. Trailer park owner wanted the fence so Huwer left it. Several poles were present on site during the 11/21/06 inspection. Tank was removed. 8 pipe was cut below the surface, plate/lid with well ID was used to cover the well. A small pile of dirt on the property was used to cover to well location, and area was seeded and fertilized back on 10/4/06. Grass shots have emerged. Photos taken of the restored area. At the time of the 10/4/06 inspection, cement did not circulate to the surface. Bob Huwer in phone meeting with him on 10/11/06 indicated that he topped off the well with 9 sack grout back on 10/4/06. |
