0430749550 2014-03-10 Production Wells No Violations During my inspection I found a pump jack well with all proper gauges and bull plugs. The lease road is stable and there is I.D. signage on the fencing surrounding the wellhead. The tank battery is fenced and is shared with #25835, #20864 and #21061, #25837. The tank battery contains five 100 bbl production tanks, three 50 bbl poly brine tanks, mechanical separator and has been surrounded by a new SPCC dike. There are no violations at this time.
0475520930 2011-05-03 Production Wells No Violations FOLLOW UP TO NOV # 1304534180 There is a tubing valve at the wellhead that is venting gas. Do not vent gas at the wellhead. Plumb the gas to the stock tank. There is oil staining in the limestone behind the 2-100 bbl stock tanks. Clean up the oil staining in the limestone and on the top of the buried brine tank. I called Dennis Williams (740-622-7692) on 5-4-2011 and discussed the well.
1175634887 2013-01-08 UrbanProduction Wells No Violations I inspected the Charles Newton 1 (34151208640000) well on January 8, 2013. My inspection was a follow up for an existing Notice of Violation (NOV) issued by inspector Dave Claus on May 3, 2011. The well was causing pollution and contamination and there were general safety concerns. There was a tubing valve at the wellhead venting gas. The gas has been plumbed to the stock tank. The oil staining on the brine tank has been cleaned. All issues from the May 3, 2011 NOV have been resolved. During my inspection, I noticed a few small holes in the stock tank and oil staining on the side of the tank. The belts on the motor for the pumping unit are broken and the pumping unit was not running at the time of my inspection. Load lines for stock tanks do have bull plugs installed but do not have locks. Locks are required due to Perry Township being an urban township. I called Discovery Oil & Gas and discussed the need to repair the tank, clean the oil staining from the stock tank, install locks on all load lines, and replace the belts. I spoke to Mike Ellenis. Mike said he was planning to replace the tank and would resolve the other issues and call me back.
