1354545498 2012-11-28 Work Over Reconditioning No Violations Dover Atwood Corporation is reconditioning the Muckley 1 (34151208690000) well in Canton Township. I inspected the reconditioning operations on November 28, 2012. My inspection found the well was completed in 1949 and has five casing strings (10 inch, 8.25 inch, 7 inch, 5.5 inch, & 2.5 inch). A cement bond log was completed on November 9, 2012. The original perforations (4800 feet to 4850 feet) were covered with sand to prevent cement from entering this zone. Four squeeze perforations were shot between 4735 feet and 4735.5 feet in order to squeeze cement behind the 3 inch casing string and into the annulus of the 5 inch casing. 70 sacks of 50/50 Poz cement was pumped through the squeeze perforations and displaced to 4700 feet using 40.9 bbls of water. The cement inside the 3 inch casing was drilled out and all the sand was flushed from the wellbore. A second cement bond log completed on November 28, 2012 showed the 70 sacks of cement covered the interval from 3230 feet to 4735 feet. 15 new perforations were shot between 4802 feet and 4822 feet and the well was fractured by Universal Well Services.
1459025794 2013-12-03 Production Wells No Violations I was inspecting this well for complaint #4588. Mr. Brian Ickes called Bob Roush to inform him of a petroleum smell and film in his water. Water is supplied by water well. First noticed spring 2013. Noticed film on his softener brine tank on 11/27/2013 Dover-Atwood did recompilation on Muckley #1 3415120869 on 11/12. Well is 879 west of Mr. Ickes home. Mr. Ickes water well is approximately 279 deep. The water well GPS is 40.74096 -81.34290. The water sample that he showed me had an oil like substance on it that would be consistent with a light weight used motor oil. Mr. Ickes relayed that the stock tank for the Muckley #1 3415120869 was replaced recently. The lease road is well maintained and the well has proper signage. The tank battery has a good SPCC dike and a new 100 barrel production tank and a new 50 barrel plastic brine tank. It also contained a vertical separator. The well has a pumpjack on it and the well has been recently worked over and reconditioned. There was no annular pressures showing on the surface casing set to 539. There was a house gas connection on the well with a positive shutoff. I conducted a quanta on Mr. Ickes water from the sink showed 28ppm of chlorides. This well is currently producing and has no volitions at the time of inspection.
