0129037495 2015-07-28 Production Wells No Violations I was on location to follow up to complaint #5076 regarding possible oil in a water well. I met on location with Enervest Foreman Bob Wayle and a bobcat operator to excavate the 8.625 diameter surface casing that is buried. The wellhead was excavated showing that the 4.5 diameter production casing is on a casing hanger. The casing hanger was opened and exposed to the atmosphere and did not flow any gas or fluid. The 8.625 diameter casing is static and does not have any pressure on it at this time. The casing hanger and wellhead are in good mechanical conditional and the well does not appear to have any mechanical integrity issues at this time.
0793144034 2015-10-13 Plug / Plug Back No Violations I was on location to check the status of reclamation for this plugged well. The top cement plug was tagged on 10/12/2015 at 65. Quikrete was brought onto location and the well was topped off. A bulldozer was on location grading the location flat and preparing for final restoration. When the location is reclaimed and vegetation established final restoration will be passed. The compliance notice to plug this well has been closed.
1401110113 2015-10-09 Plug / Plug Back No Violations I was on location to witness the plugging of this well by Chipco LLC. The 4.5 diameter casing was pulled to 3370 after the casing was shot off. A 300 86sk Big Lime plug was pumped and displaced with 49bbls of brine. The well circulated and the 4.5 casing was pulled to 1106. A 60sk 200 bottom of surface casing plug was pumped and displaced with 14.6bbls of fresh water. The 4.5 casing was pulled to the surface covering the Pennsylvania formation. A 60sk 200 plug was pumped and circulated to surface. The casing was pulled from the well and the cement top monitored. The cement continued to fall and the plug was twice topped off. Cement would not stay at surface. The plug will be tagged on 10/12/2015 and will be topped off with quickrete. When cement is brought to surface a plate with the API# will be welded onto the casing after it is cut below grade.
1444220539 2015-10-06 Plug / Plug Back No Violations I was on location to witness the Chipco LLC service rigging up on location to plug this well. The lease road and location have been cleared with a dozer and the wellhead is now accessible. A tree is overhanging the location and preventing the derrick from being raised on the rig. A dozer operator will remove the tree on 10/7/2015 to allow the rig to be raised and the plugging of the well to commence.
1493316184 2017-04-27 Final Restoration No Violations I conducted a Final Restoration inspection on this well. The site has been graded, vegetation is established and there is no active erosion. The Final Restoration is passed. The location on this well incorrectly plots west of I-77 approximately 5.8 miles west of the wells actual location. The coordinates from the plug report were used to get the wells location.
1590305938 2015-07-14 Production Wells Violations Noted I was on location to follow up to complaint #5076 regarding possible oil in a water well. I met on location with Enervest Foreman Bob Wayle to locate the wellhead. There is no associated tank battery, flowline or lease road for this well. The wellhead consists only of 4.5 diameter casing with a shut in valve on top. The 4.5 diameter casing currently has 400psi of pressure on the casing. The 8.625 diameter surface casing is buried. No production has been shown for the well since 2006. There is no access to the location to expose the wellhead at this time. Enervest will work to gain access to the wellhead and will expose the 8.625 diameter casing to allow for the monitoring of the 4.5 casing annular pressures. This well is incapable of production and shall be plugged per Ohio Revised Code 1509.12(B) within 180 days.
1936782391 2015-10-08 Plug / Plug Back No Violations I was on location to witness the plugging of this well. The Chipco service rig ran 1.5 diameter tubing to 4615and a 38sk 500 Clinton Sandstone isolation plug was pumped and displaced with 10.2bbls of fresh water. The sandline on the service rig is damaged and it cannot be used to rip the casing. Appalachian Well Services was brought on location and the 4.5 diameter casing was shot off at 3500. A joint of casing will be pulled and plugging will continue on 10/9/2015.
2072638048 2015-10-07 Plug / Plug Back No Violations I was on location to check the status of plugging this well. 2.375 diameter tools were only able to reach a depth of 3500. 1.5 diameter tools were run into the hole and were able to reach a depth of 4100. It was decided that 2.375 diameter tubing be taken off location and 1.5 tubing be run into the well with a mud anchor to try to reach bottom. The tubing hit an obstruction at 4615. The tubing was pumped on, rotated and reciprocated with no gain in depth. It has been determined that the casing is collapsed. A Clinton isolation plug will be set on 10/8/2015 and the casing will be shot off.
