0165360726 2019-02-07 Production Wells No Violations My inspection found the well is not plugged as requested by the Compliance Notice issued on 09-19-2018. The record shows no Plug and Abandon permit being issued at this time. The underground utitilties have been marked on the property where the well is located. The flowline going to and away form the tank battery has been flagged. I called Tom Vosick (M & R) (330-432-4869) and left a message on his voicemail requesting a return call.

This is swab well with a plugged flange. There are several section of flowline protruding from the ground round the wellhead. The tank battery consists of flowline and an inground concrete brine vault. There are rods lying on the ground just outside the dike of the tank battery. I spoke with Chris Belch (M & R) and he advised me fourteen wells are to be plugged this year but could not confirm if this was one of those wells.

1656392189 2016-09-19 Production Wells Violations Noted I conducted a status check inspection on this well. The RBDMS record shows this wells last year for oil and gas production was in 1989 for 219 MCF of gas and no production reports being submitted from 1990 through 2003. The record also shows zero production from 2004 through 2015. The well has a swage on the 4.5 inch casing with a closed valve and a plug. The tank battery consists of an inground concrete brine tank, flowline and some rods located inside the dike. On the north side of the dike, there are more rods on the ground. The well is not legibly marked. This well presently is incapable of production. Consequently, this well shall be plugged in accordance with Ohio Revised Code section 1509.12(B). The well owner/operator shall submit a written plugging schedule to Shawn Murphy at 3575 Forest Lake Dr., Ste. 150, Uniontown, Ohio 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
