1206627355	2008-03-27	Production Wells	Violations Noted	I received a call from Kyle Tharp (New Philadelphia office # 330-339-2207) regarding a gas leak he discovered at the well head while conducting a pre-mining permit inspection of the property. I located the well which consists of a 4 to 2 swedge plumbed into a 2 to 1 swedge with 1 valve connected to a house gas line. There was no evidence of a leak at the time of my inspection. This property is being permitted for coal mining. I will contact Ron Rocco (phone # 330-868-7055; address 15816 Crowl St., Minerva, OH 44657), the domestic well owner, and request that he post an I.D. sign at the wellhead and that he mark his house gas line so that it will not be disturbed when the mining activity begins. He should also check all wellhead fittings for leaks.
