0897954563 2016-08-31 Production Wells No Violations I conducted a status check inspection on this well. This well is equipped with a plunger-lift. The RBDMS record shows the well has a production report submitted for 2015. The diked tank battery consists of one 100 barrel tank, an inground concrete brine tank, and a horizontal separator with a meter. The well and tank battery are legibly identified. There are no compliance issues noted at this time.
1484748876 2017-01-17 Production Wells No Violations I received a phone call from Scott King (Emergency Operations & Response Manager ODNR) at 10:46 AM concerning an oil release from a stock tank for Titan/ Atlas well # 34-151-2-3422 -0000 (M & G Coal #3). I arrived on location at 11:32AM and clean-up was already started. I met with Sanford Nisly (Supervisor RWS) on location and he advised me he had shut in the well. Rusty Morrison (Atlas Production Foreman) arrived on location at 11:46AM. The leak is located ~ 7 ft. up on the side of the stock tank. The fluid from the tank leaked into the dike area of the tank battery and flowed out with the water via a 4 inch pvc pipe which was not plugged when the leak started. A plug was placed prior to my arrival. The light oil and water flowed down the lease road/ driveway ~ 600 ft. then flowed east off the driveway. There are two ponds located southeast of the tank battery and neither one had any visual signs of oil contamination. A boom was placed where the fluids exited the road as clean-up operations proceeded to the tank battery. I spoke with Sanford Nisly and Rusty Morrison and advised them the well cannot be placed back into production until the tank is repaired or replaced.
