0690738357 2009-04-01 Production Wells No Violations I received a complaint call from Jeff Miller (330-248-3996) regarding a breached tank battery dike that was discharging fresh water onto his property. I met with Jeff Miller and then inspected the site. I found that the dike was breached in two places and rainwater was flowing across a limestone driveway onto Jeff Millers property. I called Matt Pettigrew (Petrox) and requested that the dike be repaired. On 4-2-2009, I received a call from Ron Hammen, who will be repairing the dike on 4-3-2009.
1186082887 2007-08-01 Production Wells Violations Noted Clean up and remove the stained soil and free oil located in the area (12 x 6) around the wellhead. I called Aaron Pettigrew at Petrox.

The tank battery consists of a Steel production tank, a steel brine tank, a vertical separator and a meter. The well is legibly identified. No compliance issues are cited at this time.
