1282775310 2018-02-06 Production Wells Violations Noted I was on location to investigate Complaint #5781, (Idle Well). The 4.5 inch casing has a 4 inch to a 2 inch swage on it, a ball valve attached to the top of the swage is closed. The flow line is disconnected from the production casing. The tank battery consists of two steel 100 barrel production tanks and a plastic brine tank. The well is not legibly identified. The record shows the well last produced 71 barrels of oil in 2005. There are no annual production records shown from 2006 through 2009. I spoke with the complainant and informed them of my findings. I also spoke to Tim McConaha (Enervest), and informed him of my findings and that a compliance notice to plug the well will be issued. This well presently is incapable of production. There has been no reported production for 10 years. Consequently, this well shall be plugged in accordance with Ohio Revised Code section 1509.12(B). The well owner/operator shall submit a written plugging schedule to Shawn Murphy at 3575 Forest Lake Dr., Ste. 150, Uniontown, Ohio 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
1322601742 2019-01-11 Production Wells No Violations My inspection found the well has not been plugged as requested by the Compliance Notice issued on 02-06-18. A Plug & Abandon Permit was issued on 03-08-2018. This is a swab well and it is capped. The tank battery consists of two steel production tanks and a plastic brine tank. Groundwork has been around the tank battery in order to prep for its removal. The well is scheduled for plugging in 2019. Complaint #5781 is associated with this well. I spoke with the complainant and updated the complaint log.

The record shows a Plug & Abandon permit being issued on 3-8-18. This is a swab well which is capped. The tank battery consists of two steel production tanks and a plastic brine tank. I spoke with the complainant and update the complaint log for complaint #5781.
1791516012 2016-03-03 UrbanProduction Wells No Violations I was on site to conduct an inspection of this well at the request of the landowner, Daniel Miller. The landowner requested an inspection due to the well not producing and inquiring about plugging. Production on this well was last reported in 2005 with 71 BBL of oil and zero production to date. The pumpjack has been removed from the wellhead. The wellhead consisted of a 4.5 inch swage with a valve. The valve at the wellhead was closed. The electric lines and flow line have been disconnected from the wellhead. The tank battery consisted of two, 100 BBL production tanks, a 50 BBL poly brine tank and two drip bottles. The flow line between the production tanks had a leak at the union that was connected to the tank valve. The oil staining was approximately 6 inches by 10 inches and contained within the tank battery. The tank battery was not diked. The signage at the tank battery was not up to date with the current owner information and the well had no signage. The lease access was stable and driveable. I contacted Doug Bieghle at Enervest who informed me that the well was not capable of production. It is being added to Enervests plugging list. The oil stained soil will be removed and union repaired. I will follow-up in two weeks from this inspection relating to clean-up efforts.
