0459023440 2004-05-04 Production Wells Violations Noted 5-3-04 Mike Allen (North Coast) 740-432-7359 reported a stuffing box leak of approximately 3 bbl.

5-4-04 Inspected the site. The release area is 10 wide and extends 140 to the west. The release has been coated with peat sorb and oil booms have been placed on the grass at the western extent to stop any runoff. I met with a North Coast employee at the site to discuss additional clean up. Additional peat sorb will be placed on the release. Heavily contaminated soil will be removed. A stoned lease road may be placed over the area of the release after it is cleaned up. The property owner is aware of the situation.

1087565672 2004-06-17 Production Wells No Violations Cleanup is not completed. Called Jeff Murrey (North Coast) to discuss additional remediation. I also met and discussed the situation with the property owners neighbor.

1310667309 2011-07-14 Production Wells No Violations I received a call from Brian Levengood (330-323-2021) at 0446 hrs reporting a plastic flow line leak that sprayed oil over a 60 long x 40 wide area in a cornfield to the south and adjacent to the pumping unit. The well was shut down and the site remediation will begin at daylight. When I arrived at the site, all of the stained soils and vegetation had been removed. I met with Mike Miller, the homeowner to the north, who receives house gas from the well. He said that he heard the line rupture, ran out, shut down the pumping unit, and then called the fire department. The release site has been remediated. A new steel flow line will be installed.
