0091239040 2007-06-27 Preliminary Restoration No Violations I inspected this site for Preliminary Restoration and met with the property owner, Jim Henson. Preliminary Restoration is due 9 months from the date drilling began. This well was spudded on 9-29-2006. Preliminary Restoration is due 6-29-2007. There are two areas of concern at this site. There is a barren area (80 x 20) located west of the pumping unit that was formerly the drilling pit. It is rough graded and has pieces of black plastic pit liner sticking up through the soil. This area needs to be graded and have additional topsoil brought in, regraded, and seeded. The other barren area (20 x 20) is located east of the pumping unit near the tank battery dike and the creek. This area has been used as a borrow area for soil and is now lower than its original elevation. This area needs to be filled in with additional topsoil, graded, and seeded. Jim Henson and I discussed these two areas at length and he has agreed with my recommendations. Once this reclamation is completed and vegetation is established, I can pass the Preliminary Restoration on the site. Jim Henson also has some concerns about two trees that were damaged and one tree that was knocked down into the creek. The tree damage needs to be negotiated between Great Lakes and Jim Henson. If necessary, I can be on the location when the additional reclamation is being done. I informed Nick Berelsman (Great Lakes) of the findings of my inspection and I will mail Carl Von Allman (Great Lakes) a copy of this inspection report.
1179861281 2007-05-22 Preliminary Restoration No Violations I received a call from Attorney Jeff Fickes (330-665-5117) regarding the reclamation of a drilling site that was located on his clients property on Liberty Church Rd. in Paris Township. Great Lakes Energy will be getting a third party contractor to do the Preliminary Restoration on this site.
1426841424 2007-04-26 Preliminary Restoration No Violations The well has been completed as a pumping well. The location is reclaimed. Vegetation is sparse. I did not pass the Preliminary Restoration at this time.
