0671365997 2015-04-29 Production Wells No Violations I was on location to follow up to the final remediation of the brine spill for this well. The sump holes have been vacuumed out and the location has been flushed with a load of freshwater to flush any residual brine from the location. The water was vacuumed and taken to an injection well. A total of 30 tons of dirt have been excavated and taken to the Kimble Landfill for disposal. The track hoe was hooked up to the pumpjack and straightened over the wellbore to prevent any future stuffing box wearing out prematurely. A dike around the wellhead was constructed and the location seeded. The tank battery was cleaned up and the identification made to be more visible. At this time the brine spill has been remediated and the well made to no longer leak. There are no violations at this time.
1084141493 2016-08-23 Production Wells No Violations I conducted a status check inspection on this well. The well is equipped with a pumpjack with an electric motor and there is a domestic gas connection at the well. The RBDMS record shows the well has a production report submitted for 2015. The well is shut-in. The diked tank battery consists of one 210 barrel tank, one 50 barrel brine tank, and a vertical separator with a meter. The well is legibly identified. There are no compliance issues noted at this time.

1348624424 2015-04-28 Production Wells No Violations I was contacted by Troy Wells of Enervest at 14:20 indicating that a brine spill had occurred at this well. Joe Little Excavating has been called to the location to excavate and remediate the location. The pumpjack well was set on the well at an angle and the polishing rod was causing excessive wear on the stuffing box. The stuffing box failed and a maximum amount of brine that spilled was 5bbls during the production period. A 60x3 kill area is present near the tank battery from the spill. A 15x10 kill spot is present near the wellhead and the brine migrated down the fence line of the pasture causing a 90x3 kill spot. No roadways or waterways were impacted. The kill spots were excavated and disposed of at the Kimble Landfill. While excavating the soil around the tank battery it was discovered that a clay and plastic liner from the drilling of the well has trapped the brine in the gravel on location. Two sump holes were dug and the stone excavated. Six bags of dolomitic lime were spread on the spill area and flushed with a load of fresh water. The water was vacuumed up and brought to an injection well. The excavated soil will be left exposed overnight and the area flushed again in the morning to complete the remediation.

A new dike has been built around the pumpjack well and the contaminated soil has been removed and disposed at the Kimble Landfill. There is no evidence of the spill on location and there are no violations at this time.
