The compressors are running and pumping gas downhole, bringing oil to surface. The system will be monitored for several days to ensure smooth operation and determine if any changes to the system are needed.
0184338032 2015-03-02 Production Wells No Violations I was on location to check the status of determining whether or not downhole valves are leaking on the tubing string. A wireline truck utilizing pressure and temperature gauges has been checking 50 before, on the valves, and 50 after each valve present in the string. The gauges gather the information downhole and cannot be analyzed until it is pulled from the well. The well is producing during the measuring and testing of the well. The testing will continue into tomorrow.
0235462072 2014-08-25 Drill / Deepen / Reopen No Violations I was on location to check the status of Pep Drilling this well. Currently the rig is drilling ahead at 2561 with a 7.865 hammer bit. I walked the location and new erosion controls have been installed following heavy rains seen last week. The pits are in good condition and there are no issues with the liners. The reserve pit is currently being drawn down by Mac Oil water trucks to handle any possible water encountered in the Oriskany Sandstone. They will drill on air until they reach their mud up point and load the hole with saturated brine. There are no violations at this time.
0249970759 2015-02-23 Production Wells No Violations I was on location with District Supervisor Rick Anderson to follow up to complaint #4943, in which the complainant stated that exhaust from compressors installed at the well site are causing odor issues at the complainants residence. On location for the inspection were Carl Safreed with the Stark County Health Dept. and Tim McConaha and Charlie Youngen of Enervest. An evaluation of the location determined that a vent stack coming from the dehydrator used to remove moisture from the natural gas stream was the source of the odor. The compressors at the well site are equipped with hospital grade mufflers and are not the source of the emission odor. A charcoal filter was installed on the vent on the dehydrator, adsorbing the emission odors. Enervest has been advised to check and replace the charcoal filter regularly to prevent odor breakthrough. The odor source has been eliminated at this time and the complaint can now be closed.

Currently the Chipco LLC service rig is running in the last of the rods for the well. The pipelines, electric and tank battery have all been installed. A Chipco LLC service rig will be location 3/27/2015 to trip tubing out of the well in preparation to remove the gas lift production system. A Dansco pumpjack has been purchased and will be installed on the well for production. The compressor station will also be removed as it will be no longer needed for production of this well.
0642429488 2014-10-24 Completion Testing No Violations I was on location to follow up to complaint #4843. Complainant stated that as he was traveling on Route 62 by the Rte 44 exit when he smelled a very strong odor of gas. He stated that there had been drilling equipment there and that he thought that may be causing the odor. He said he has smelled this for the past week and wanted to call to make sure someone looked into this. The smell is gone and no longer an issue. There are no elevated gas levels and the complaint can now be closed.

Pep Drilling drilled a 17.5 diameter hole to 40 and a 14.75 diameter hole to 525. They installed 505 of casing with 4 centralizers and a guide shoe. Universal pumped 370sks of Class A 3%CaCl cement and displaced it with 57.8bbls of freshwater. 13bbls of good cement were returned to surface. The rig will wait on cement for 8 hours before drilling ahead. There are no violations at this time.
0801733535 2015-09-29 Preliminary Restoration No Violations I was on location to pass preliminary restoration for this well. The lease road is gravel and stable. The electrically driven pumpjack has all required valves and gauges and is identified. The tank battery is surrounded by a dike and contains a vertical separator, four (4) 210bbl production tanks and a chart box. The roadway, tank battery and wellhead all conform to spacing requirements. The wellhead and tank battery are properly identified and contain required locks. The location has been reclaimed and vegetation has been established. At this time preliminary restoration has been passed.

The rig was drilling at 7320 and has approximately 80 more to go. A load of new 17 lb/ft P-110 5.5 OD production casing was being delivered at the time of inspection. They expect to TD the well and begin running casing in the early morning.
1409769701 2014-09-03 Drill / Deepen / Reopen No Violations The Pep drilling rig is currently shut down due to the failure of their mud pumps. Drilling has been suspended for 7 hours and the pumps are currently brought back on line. They are in the hole at 5775 and circulating the hole while the pumps are partially functioning. There are no violations at this time.
1409924869 2014-09-04 Drill / Deepen / Reopen No Violations I was contacted by Joe Wade of Enervest (330-542-1705) informing me that they have a hole in the liner of their working pit for this well. They have seen a bubble forming on the backside of the pit and it appeared it was a large quantity of rain water. When the water was sampled it was discovered to be brine. They pumped all the brine from the backside of the liner to an additional pit and began pumping the entire pit dry. Once the brine and water on the backside of the pit was pumped out and the brine in the liner was pumped out into frac tanks, the liner was pulled back and out of the pit. An excavator then scraped out all of the excess brine and contaminated soil from the pit and put into the reserve pit. Once all the brine and contaminated soil was removed the pit was re-lined and the pits refilled. Once this is done the rig will begin drilling again. There are no violations at this time.
1410383511 2014-09-10 Urban Drill / Deepen / Reopen Violations Noted

--110 casing in the horizontal section of the wellbore. While collecting samples from the top of a frac tank; salt crystals were noticed near several puddles on the drilling location between the pits and the frac tanks. The gravel west of the frac tanks was also darkly stained and indicated a brine spill. Hach High Range Quantabs (bottle number 4190), were used to test the puddles and water near the frac tanks and east of the frac tanks for chlorides. The readings exceeded the maximum obtainable range of the Quantabs, indicating chloride levels above 6000ppm. The affected area east of the pit was approximately 75x75 and the area with the salt crystals between the frac tanks and pits was 30x45. Enervest was informed of the spill and told to immediately bring on location excavating equipment and vacuum trucks to vacuum and excavate free brine and impacted soil on the location. Joe Wade of Enervest (304-542-1705) stated that after speaking with staff, it was determined that the spill was caused by personnel allowing a brine truck to overfill while pulling water from the pits. The weather forecast for the day called for heavy rains. Any puddles that accumulate on location must be tested for chloride concentrations and if elevated must be vacuumed from the location for proper disposal. Additionally, the affected soil and rock must be excavated and disposed of properly. All free brine on location must be vacuumed, and the location modified to not allow any brine to leave location during the rain events. This spill must be immediately addressed and fully remediated to the before this compliance notice will be closed.

1410870975 2014-09-11 Urban Drill / Deepen / Reopen Violations Noted I was on location with District Supervisor Rick Anderson to check the status of the cleanup for the brine spill that was discovered 9/10/2014. There was a rainfall event the night of 09/09/2014 that added water to the surface of the location and leached brine from the contaminated soils. There were several puddles of water located near the rig floor and mud pumps that had elevated chloride levels ranging from 1100ppm to 3000ppm. A vacuum truck was brought onto location to vacuum all free water and brine on location. Impacted soil was also scraped up from the areas of the high chloride puddles and disposed of into the pits. While inspecting the base of the rig substructure the mouse hole and rat hole sleeves were being loaded into tubs for transport, the location of the mouse and rat holes were identified by a Pep Drilling hand. It was noted that the mouse and rat holes were not properly sealed around the steel sleeves used to house the kelly and the drill string. Section 1501:9-1-
1410982291 2014-09-16 Casing String Witnessed No Violations I was on location to witness the cementing of the 5.5 diameter vertical production casing of this well. A cement port collar was installed in the casing at 4412 and 2 3/8 diameter tubing was stabbed into the port collar. Universal pumped 485sks of Unifill Light cement and 100sks of 50/50 2% gel, 10% salt tail cement. The cement was displaced with 15.2bbls of freshwater. A reverse circulation of 100bbls of brine and 10bbls of fresh water were used to remove any residual cement in the casing after the cement port collar was closed shut. The frac stack will be installed and the well prepared for stimulation. A track hoe was on location solidifying the reserve pit at the time of the cement job. The pit was properly solidified and had no free liquid and was in a firm solid state. The working pits are going to be closed and solidified tomorrow. There are no violations at this time.
1411496752 2014-09-15 Preliminary Restoration No Violations I was on location to follow up to compliance notices issued in inspection #1410383511 and #1410870975. At this time the brine spill has been completely remediated and water tested on location does not have any elevated chloride levels. The mouse and rat holes for the rig have been plugged with quikrete cement and are no longer open. Pit solidification will begin tomorrow and the tapered 4.5/5.5 diameter production string will be cemented tomorrow using a Chipco service rig and Universal Cementing. At this time both compliance notices have been addressed and are closed. There are no violations at this time.

There were several spots where the tubing got tight and a total of 70k pounds were pulled on the tubing. The tubing will be removed from the well and steam cleaned on location with containment later today.
1604024874 2014-08-26 Drill / Deepen / Reopen No Violations I was on location to check the status of Pep drilling. Currently the rig is drilling at 3398 in the Helderberg formation. When drilling through the Oriskany they encountered water flow and had to begin soaping the well. The air hammer failed and they tripped into the hole with a trichone bit and are currently drilling with it. The pits have been sucked down and are in good condition. There are no violations at this time.
1606865077 2014-10-23 Completion Testing No Violations I was on location to follow up to complaint #4843. Complainant stated that as he was traveling on Route 62 by the exit that takes you to Louisville, he smelled a very strong odor of gas. He stated that there had been drilling equipment there and that he thought that may be causing the odor. He said he has smelled this for the past week and wanted to call to make sure someone looked into this. I was on the side of Route 62 and smelled a noticeable amount of natural gas. A citizen was on the side of the road pulled over and stated he called 911. I contacted the Nimishillen Fire Dept and they were on their way to the location. I called Mike Burkhart and Joe Wade of Enervest who were also on their way to the location. There was no flow line or gas leak found near the side area of Route 62. Myself, Nimishillen Fire Dept and Enervest entered the Ramsey location and noticed that the wind was blowing south toward Route 62 from the open top oil tank that the well was flowing back into. A Honeywell PHD6 gas meter was used to check gas levels near the road and the location and no elevated levels were found. It was found that the wind was blowing south across the open top tank and the gas smell was concentrating near the side of Route 62. Because no elevated gas levels were found by the Nimishillen Fire Dept it was deemed there was not a safety hazard from the gas smell and when the winds shift the smell will no longer concentrate on the roadside. I will check back tomorrow to follow up and ensure the gas smell is not present.
1845796851 2015-01-29 Production Wells No Violations I was on location to check the status of getting the gas lift production system for this well installed and operational. Gas is being pumped down the well but not fluid is coming back to surface. Several adjustments were made but unsuccessful at getting the well to lift fluid. There has been trouble getting pressures to properly regulate on the system. It is believed that the cold temperatures are causing a pop off to prematurely go off at a lower then desired pressure. This is not allowing downhole pressure to build to lift the fluid present in the well. The fluid level in the well has been shot to measure its depth over several days and has not changed. Work will occur over the weekend to try to get it operational.
1955208059 2014-08-18 Drill / Deepen / Reopen No Violations I was on location to check the status of Pep Drilling rigging up on location. The rig floor has been set over the hole and the derrick is currently having some maintenance done on the pulleys and guidelines. The watertight cellar and PVC flow line has been set on location and will flow to the pits when they are dug. Currently the pits are being excavated but will not be completed until 8/19/14. They expect to spud the well 8/20/14 morning.
2069481669 2014-10-08 Completion Testing No Violations I was on location to perform an inspection on this well in regards to complaint #4822 for a water well investigation. The well is currently being prepped to perform the flow back because this well was recently hydraulically fractured, there were no issues during the frac job. The wellhead has good mechanical integrity and appears to have no mechanical issues at this time. There are no violations at this time.
