2015-11-23 | Preliminary Restoration | No Violations | I was on location to check the status of preliminary restoration for this well. The electric pumpjack has been installed and is producing into the completed tank battery. Identification is present on the wellhead and the tank battery. The tank battery is surrounded by a dike and a fence. The tank battery contains two (2) 210bbl production tanks, poly brine tank, vertical separator, chart box and production tank vapor recovery system. The stone drilling pad is still in place. I will perform a follow up inspection to ensure the remaining stone has been removed from location and vegetation has been established. There are no violations at this time.
2015-08-18 | Completion Testing | No Violations | I was on location to check the status of stimulating the well for production. Reliance Well Services was on location prepping the equipment for the frac and mixing gel. The tank battery location was being cleared and prepared for the installation of the production tanks, brine tank and separator. There are no violations at this time.
2015-07-24 | Drill / Deepen / Reopen | No Violations | I was on location to have a pre spud meeting with the Pep rig. The rig is currently prepping the mud pump to mix the freshwater drilling mud. The rig will catch cuttings in the Big Injun in order to determine the formation base. The rig expects to spud the well today. There are no violations at this time.
2015-08-10 | Drill / Deepen / Reopen | No Violations | I was on location to check the status of Pep Drilling moving from this location to a location in Tuscarawas County. The derrick has been dropped and trucks are on location moving equipment. The closed loop pit system is still on location. The cuttings have been solidified and disposed of off location. A service rig will be on location 8/12/2015 to perform the cement job that covers the Utica/Point Pleasant and Clinton Sandstone formations. There are no violations at this time.

2018-04-16 | Production Wells | No Violations | I did a status check inspection for this well. There were concerns about the production tank being to close to the nearest house. Using a 100 foot tape measure, I measure the distance to the house from the nearest production and the vertical separator. I measured 101.2 ft. from each. I contacted Rob Lowe (Oil & Gas Survey) he advised me Lucas Smith will take measurements this week. The well is equipped with a pumpjack. The tank battery is equipped with two steel production tanks, a vertical separator, a plastic brine tank and a meter. The well and tank battery are fenced in, gated and locked. The well is legibly identified.
2015-07-16 | Surface Facility Construction | No Violations | The location for this well is currently being constructed. Geotextile fabric and stone are being placed on the location to stabilize the floodplain soils. The location will continue to be built and the Pep Drilling rig will move onto location when they are done drilling the Burns Unit 1K well. This well will be drilled using a closed loop steel pit system.
2015-07-27 | Casing String Witnessed | No Violations | I was on location to witness the cementing of the 8.625 diameter intermediate casing. The Pep drilling rig drilled a 11 diameter hole to 1037 and 1017 of 24lb/ft 8.625 diameter casing was installed in the wellbore with a float shoe and centralizers. Universal pumped 350sks of Type 1 3% CaCl cement at 15.6ppg. There was full circulation throughout the job and 20bbls of cement were circulated to surface. The rig will wait on cement for 8 hours before installing and testing the blow out preventer. There are no violations at this time.
2015-09-14 | Preliminary Restoration | No Violations | I was on location to check the status of preliminary restoration for this well. The electric pumpjack has been installed and is producing into the partially complete tank battery. Identification is present on the wellhead and the tank battery. The stone drilling pad is still in place and heavy machinery is on location to remediate the location. I will perform a follow up inspection to ensure the remaining stone has been removed from location and vegetation has been established. There are no violations at this time.
2015-07-26 | Casing String Witnessed | No Violations | I was on location to witness the cementing of the 11.75 diameter surface casing. The offset wells and Enervest geology prognosis indicated the Big Injun sandstone bottom would be around 290. The Pep drilling rig caught samples and it was determined from evaluating the drill cuttings and the geolograph that the base of the Big Injun was 286. The Pep drilling rig drilled a 17.5 diameter hole to 155 and a 14.75 diameter hole from 155 to 356. A total of 346 of 11.75 diameter 42 lb/ft casing was installed in the wellbore with a guide shoe and centralizers. Universal pumped 360sks of Type 1 3% CaCl cement slurry with .25 lb/sk of flake at 15.6ppg. The cement was displaced with 40bbls of freshwater and a total of 20bbls of good cement was circulated to surface. The Pep drilling rig will wait on cement for 8 hours before drilling ahead to 1022.
2015-06-29 | Surface Facility Construction | No Violations | I was on location with Mike Burkhart, Charlie Youngen of Enervest and John Popovich of Pyramid Pipeline to drill test holes on location to check groundwater. The location is located in a floodplain and a request to waive the steel pit requirements was made. A test hole was dug in the location of the pits and water was encountered at 3.5 below ground level. Due to the concern of flooding and groundwater intrusion the request to waive steel pits has been denied. Steel Pits must be utilized in the drilling of this well.
2015-08-05 | Drill / Deepen / Reopen | No Violations | I was on location to check the status of the Pep Drilling rig. The rig is drilling the 7.875 diameter hole at 7250 on fluid. The rig expects to reach total depth tonight. The cutting from the well have let Enervest determine that they will set and cement production casing on this well. There are no violations at this time.
2015-07-22 | Surface Facility Construction | No Violations | I was on location to check the status of building this location. The steel pits have been placed and the gas buster has been placed 60 from the wellbore. Welders are on location installing plates into the pits to help with the settling out of cuttings. Gravel has been spread and the location containment is intact and set. The rig crew will take 7/23 off to allow for the final welding of location to be complete.
