The well site has been restored and no erosion issues were observed. The lease access has been restored. There is a plug report on file in the state database showing the well plugged on 10/07/2014. 1230619144 2014-04-07 Administrative Inspection No Violations I performed an administrative inspection in reference to Complaint #4649. I contacted Rusty, field supervisor for Atlas, concerning plugging the well. Rusty stated Atlas was in the process of filing for a new plugging permit. I will follow-up on an approximate plugging date once I receive the permit.

1362583964 2013-03-06 UrbanProduction Wells No Violations My inspection found the well equipped with a plunger lift unit and not producing. It had the proper identification posted. The tank battery contained one, 100 barrel production tank. It had a weathered dike; however, it meets containment requirements. There was a lock and plug on the load line. The locked, heat treated separator had the correct spacing from the tank. The flow line from the separator was closed and disconnected. The lease road was in good, stable condition. I researched our database and found a plugging permit issued on 1/24/2012. I called Atlas at 330-397-2094 to inquire about plugging the well. I was told this well was on a list to be plugged by the end of the year.
1396441881 2014-03-31 Production Wells No Violations My inspection was in reference to a landowner Complaint #4649 about the well being idle. The well is equipped with a plunger lift unit. The valves on the wellhead are shut in. The tank battery consists of a 100 BBL production tank. The heat treated separator has the proper spacing and is disconnected from the flow line. The well has the proper identification. The lease road is stable. The company had a plugging permit for the well and it expired in Janurary. I spoke with Rusty Morrison about the well. The company is obtaining a new plugging permit and has the well scheduled to be plugged this year.
1412702779 2014-10-06 Plug / Plug Back No Violations I was on location to witness a plug job by Nicks Well Plugging for Atlas. First, Nicks rigged up and pumped 35 bbls of water to clear the 2.375 tubing and establish circulation for the bottom plug. Plug #1 was set from 4,150 to 3,827. The operator mixed and pumped 25 sacks of Class A cement. The cement was displaced with 15.5 bbls of water. Nicks removed the tubing from the well. Next, the operator ripped the 4.5 casing at 2,930. The well was shut-in for the night.
1412943421 2014-10-27 Plug / Plug Back No Violations I was on location to witness Nicks Well Plugging plug the well for Atlas. First, Nicks pulled 5 joints of 4.5 casing for the Big Lime plug. Plug #2 was set from 2,753 to 2,453. Nicks broke circulation with 10 bbls of water. The operator mixed and pumped 86 sacks of Class A cement. The cement was displaced with 39.7 bbls of water. Next, Nicks removed 70 joints of 4.5 casing for the surface casing plug. Plug #3 was set from 440 to surface. The operator mixed and pumped 135 sacks of Class A cement. I witnessed good cement returned to surface. Nicks pulled the final 13 joints of casing and topped off the well with cement. A total of 86 joints of 4.5 casing and 4,150 of 2.375 tubing was removed from the well. The cement sample was collected and weighed.
