1517408815 2018-01-26 Production Wells Violations Noted I conducted an inspection on this well site as a request from Mr. Tom Schwartz. Mr. Schwartz was looking to purchase the land and was inquiring about current well ownership, and production status on this well. The well was equipped with a plunger lift assembly and the annulus valve and 2 inch flow line was closed. The well was overgrown with small brush and trees. Production for this well was last submitted in 1986 meeting state requirements. All other production submitted through 2016 was recorded as zero. The tank battery consisted of one 100 BBL production tank and a heater treater separator. Valves on the separator were closed. The tank battery was diked and overgrown with brush and small trees. There was no signage at the well or tank battery locations. The lease access was overgrown and not stable. I contacted Fran DeSanctis from Petrox regarding the well ownership and production status. She confirmed that Petrox does own the well and would forward the inspection information to Petrox management.

Required remedial action:

1) Install signage at the well site location. 2) The well owner operator shall also submit a written plugging schedule to inspector Shannon Mullins at 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 /within 30 days. Per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging is approved or denied.

1544109881 2018-09-27 Plug / Plug Back No Violations I arrived back on location and Nicks Well Plugging was mixing cement to spot the Berea Sandstone/Ohio Shale plug from 549-349. 12.8 barrels of cement slurry was pumped and displaced to 349 with 1.4 barrels of water. The tubing was pulled to a depth of 200 and NWP rigged up and mixed and pumped cement until good returns were achieved to the steel circulating tank. The tubing was pulled and NWP began to rig down and move equipment off of location. I checked the top plug two (2) hours after being spotted and it was three (3) inches below the top of the casing head and beginning to gel. There were no observable leaks and I approved the cutting off of the 8.625 surface casing head. See plugging report for more details on this job. No violations.

1701714159 2018-09-18 Production Wells No Violations Follow-up to Compliance Notice #1517408815 issued for failure to properly identify the well and requiring that the well be plugged. I received a call from Jason Nicholas (Nicks Well Plugging) this morning to inform me that they would be moving in on the afternoon of 9/19/2018 to get rigged up to plug this well and the Michetti #2 (PN# 21263) to satisfy the Compliance Notice. I inspected this well today and found the location has been partially logged out and the lease road is semi-stable. The wellhead consists of 4.5 production casing, 2.375 tubing, and a plunger lift system. The gate valve and the valve on the flow line are closed. The tank battery consisted of a heated, horizontal separator and one (1) 100 barrel tank. The required ID has not been posted at the tank battery or at the wellhead. This violation continues.
1967555377 2018-09-27 Plug / Plug Back No Violations I was on location to witness plugging operations on this Clinton Sandstone well. Nicks Well Plugging (NWP) was rigged up and ran 2.375 tubing to a depth of 3196. They rigged up to the tubing, established circulation, and then mixed and pumped 18.1 barrels of cement slurry. They displaced the cement to 2896 with 11.2 barrels of water and the plug was emplaced across the top of the Big Lime. See plugging report for more details on this job. No violations.
