0529728429 2019-01-31 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice #1495639844 and complaint # 5609. The well was equipped with a plunger lift assembly and the annulus and flow line valves were open. The site was snow covered which was not allowing me to see if the well was still leaking gas from the 8.625 inch casing. Big Sky has not filled in the hole around the wellhead. The hole was full of water and snow covered. A Vortex Energy LLC identification sign is posted at the well. The tank battery consisted of one 100 BBL production tank, one 50 BBL tank and a mechanical separator. The flow line ball valve and sales line ball valve at the separator are open. The well is still registered under Big Skys name, but the chart on the sales line dated 12/7/17 had Vortex Energys name on it. A Vortex Energy stenciled identification sign was also still present at the separator. Access to the tank battery and well was stable.

1233824530 2016-04-18 Production Wells No Violations I conducted this inspection to follow---105 requiring Big Sky Energy Incorporated to suspend all oil and gas operations in the State of Ohio. My inspection found the wellhead equipped with a 1.5 plunger lift system. The hammer unions on the casing and tubing flow lines as well as the flow line tee have oil staining and need to be tightened. No oil has reached the ground surface. The gate valve and flow line valve at the wellhead are open and the casing valve is closed. The well is producing to a horizontal separator and the flow line and sales line valves are open. The 31 day production chart in the separator was changed on 2-29-2016 and is currently recording production and overprinting previous production data. The electronic controls are on and set to cycle the well in five (5) hours. The well produces to a diked tank battery containing one (1) 100 barrel production tank and one (1) 50 barrel brine tank. The 50 barrel tank is disconnected from service. The ID on the tank remains legible, but is very faded. Big Sky Energy Incorporated has failed to comply with the above orders to suspend oil and gas operations in the State of Ohio and shall immediately cease the production operations of this well as required by the temporary restraining order issued by the Trumbull County Court of Common Pleas, dated 4/11/2016.
1383766850 2013-11-06 Production Wells No Violations I responded to complaint #4530 listed as a brine leak. My inspection found the 4 load line plug on the production tank slowly dripping brine. I called Bob Barr, Jr. at 740-624-0434 and informed him of the complaint. Bob stated a well tender would respond to tighten the plug.
1389840781 2012-12-11 Production Wells No Violations I completed a production well inspection status check. My inspection found the well equipped with a plunger lift unit and located southwest of the tank battery. The tank battery contained one, 100 barrel oil storage tank with a stenciled identification sign, one, 50 barrel brine storage tank, and a weathered dike. The heat treated separator was located outside the tank battery that appeared in good working condition. The lease road was in good working condition. I noticed no visible spills or leaks.

-up line. There is a stenciled identification sign on the 100 bbl. storage tank with Big Sky listed at the well owner. The separator is outside of the dike. There is a stenciled identification sign on the separator. The well owner on the stenciled identification sign is Vortex Energy LLC. There is a deduct meter and 31 day chart dated 4/2/17 inside of the separator. The access road is in good repair. Photos were taken and are available upon request. I called and left a message with Vortex Energy for a return call concerning the well. Next, I spoke with Charlie Momosis, contractor well tender for Vortex Energy. Mr. Momosis stated he knew about the gas leak at the well. Mr. Momosis stated he is currently working on another well and plans to move the rig onto this well as soon as possible. Mr. Momosis stated he would stop by the well to blow down the well through the separator. I told Mr. Momosis follow-up inspections will continue until the gas leak is repaired.

A compliance notice has been issued: Big Sky needs to repair the gas leak at the well by June 2, 2017.

The 8-5/8 casing was put in line to sell down to line pressure 50 / 60 psi. Charlie will install a pressure gage on t 8-5/8 casing. Inspector Rick Skidmore will monitor the 4.5 casing pressure.
