1343996941 2012-08-01 Plug / Plug Back No Violations On 8/1/2012, I witnessed the start of a plug job by J-Well for Kleese Development Associates. My inspection found that J-Well was unable to pull up on the 1.5 tubing indicating a possible salt blockage. Matt Kleese pumped fresh water into the well to break through the blockage with brine circulating back to the surface. However, the tubing was still hung up. The operation was shut down for the day and will work on the tubing tomorrow (8/2/2012).
1614929155 2012-08-03 Plug / Plug Back No Violations On 8/3/2012, I completed a plug job performed by J-Well for Kleese Development Associates. The tubing was shot off at 2,391 and 71 joints were removed, all of which appeared wavy. The 4.5 casing was shot off at 2,391, one joint was removed and a plug was set. A total of 68 joints of 4.5 casing were removed from the well. Plugs were set at the Big Lime, base of the surface casing, and to the surface. Please refer to the plugging reports for more details.
1707945169 2012-08-20 UrbanProduction Wells No Violations On 8/20/2012, I met with Matt Kleese of Kleese Development Associates and the landowners representive, Marty Cisine, to discuss the reclamation of the access road to the permitted wells 2-2626 and 2-2625. I was informed by Marty that he had improved the access road from the tank battery to the curve, past well number 2-2626, prior to the plug job by Kleese Development. Marty agreed to allow Kleese Development to bring in a tractor to disk up the soil, sod, and small sticks from the curve in the access road located ~ 0.1 miles past well number 2-2626 to well number 2-2625. Kleese will bring in a small tractor, with a tiller, to work the access road from the prior tank battery location to the curve past well number 2-2626 and then use a rake to smooth out the access road. I instructed Matt Kleese and Marty Cisine to contact me once the reclamation was completed and I will perform a final inspection.
1991957430 2012-08-02 Plug / Plug Back No Violations 8/2/2012, I witnessed the plug job by J-Well for Kleese Development Associates. My inspection found the bottom plug was spotted through the tubing with 30 sacks of Class A cement. Then Appalachian Well Services attempted to shoot off the tubing, but encountered an obstruction inside the tubing at approximately 2,637. J-Well spend the rest of the day pumping fluid through the tubing and tried to ram through the blockage, with no luck.
