0378326205 2019-02-25 Production Wells Violations Noted I conducted an inspection on this well. At the time of this inspection, the was equipped with a Concoil plunger lift assembly. The casing valve was closed and the valve on the 1 inch flow line was open. The tank battery consisted of a 210 BBL production tank, a 100 BBL brine tank and a mechanical separator. The valve on the inlet side of the separator was closed. The valve on the sales line or outlet side of the separator was open, however at this time the well was not in production. The diked tank battery contained legible identification. The lease access to the well is shared with the landowners driveway. Production records on file show zero production from 2016 and 2017, submitted by another company, Bocor Holdings, LLC. A plugging permit had been obtained for this well initially on 10/18/16 and expired on 10/18/18, however the plugging permit has been renewed by owner Interstate Petroleum on 9/12/18 and expires on 9/11/20. To date the well has not been plugged. I contacted well owner Ken Adams who stated at this time, the well was not capable of production, due to a lodged tool within the well tubing. Mr. Adams stated that he intends to plug this well this summer of 2019. The Owner / Operator of permit 34-155-2-2640 shall submit, within 14 days, to the Division of Oil and Gas Resource Management @ 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685-8116 attn. Shannon Mullins, a plan detailing a plugging schedule for the E & J Shones #4 well. The Division of Oil and Gas Resource Management shall either approve or deny the submitted plan. The E & J Shones #4 shall be plugged within 180 days.

-in-law, were on location representing the landowner Brian Armour. Mr. Adams stated he is required by order of the Trumbull County Court to plug the well by January 3, 2017. Mr. Adams was informed that all of the production equipment and the gravel in the tank battery must be removed once the well is plugged. Mr. Armour requested that all underground piping be flushed, cut off below grade and buried. I informed Mr. Adams that a Form 5 must be submitted for the access road and all underground piping. The RBDMS database shows a plugging permit was issued on 10/18/2016.

1485268577 2017-01-24 UrbanProduction Wells No Violations At the time of my inspection, I found the hammer union near the top of the 4 plunger lift assembly still leaking gas. The absorbent pads are still in place. Oil residue is still present on the leaf litter between the absorbent pads and well. I spoke with Ken Adams, owner for Interstate Petroleum. Mr. Adams stated he is still waiting on a call back from Jeff Moore from Moores Well Service. Also, Mr. Adams stated he has contacted two (2) other well service companies. 1550234966 2019-02-08 Production Wells No Violations I was on location in regards to complaint #5622. At the request of well owner/operator Ken Adams, I conducted an inspection of the well location. Mr. Adams wanted to know if the access to the well and tank battery were open to be able to get the production tank emptied of fluid. At the time of this inspection, the well was equipped with a Concoyle plunger lift assembly and the valves on the tubing and flow line were closed. A plugging permit has been obtained for this well but the well has not been plugged. The tank battery consisted of a 210 BBL production tank, a 100 BBL brine tank and a mechanical separator. Valves on the separator were closed. The tank battery was diked. Lease access to the well was clear. I spoke with landowner Brian Armour, updating him on Mr. Adams request to come in and empty the production tank. There were no other changes to the well site location. I contacted Ken Adams updating him on the site status.

At the time of this inspection, the 210 BBL production tank heater tube has had a plug installed on it and no leak was observed. The complaint has been remedied. I spoke with Brian Armour, complaintant and landowner regarding this inspection and that the complaint would be closed.
1552574352 2019-02-15 Production Wells No Violations I conducted an inspection on this well site regarding complaint #5622, regarding a leaking heater tube on the 210 BBL production tank. At the time of this inspection, the heater tube leak has been remedied. An expandable plug has been installed in the heater tube and no leak was observed during this inspection. I spoke with landowner Brian Armour regarding the remediation that has been conducted on the leaking heater tube and the closure of the complaint.

Mr. Adams has spread mulch over the entire area impacted by the brine spill located outside of the dike on the west side. Mr. Adams had a new expansion plug to install in the heat tube for the 210 bbl. storage tank.

An identification sign was properly posted on the production tank. The dike met containment requirements. A bull plug was properly installed on the load line. The lease road was in stable condition.
