0372895842 2019-03-25 Production Wells No Violations I was location with Fred Romeo, DOGRM-Emergency Operations Coordinator, and conducted a compliance notice follow-up inspection for compliance notice # 1553259903, regarding the oil spill from the tank overflow. At the time of this inspection, no one from Vortex Energy was on location and no remediation had taken place since the last inspection conducted on 3/21/2019. The oil was still present between and under the 100 BBL and 50 BBL tanks and numerous absorbant pads were spread out over the oil. The 100 BBL tank was still full of fluid and needs emptied. The free oil and impacted soil needs removed from location and disposed of properly. I contacted Vortex Energy, LLC and spoke with field superintendent, Greg Henderson. Discussed with Mr. Henderson that the site has not been cleaned up from the oil spill and the dike has not been installed. I also discussed that the 100 BBL tank still needed emptied. Mr. Henderson stated that he thought the site had been cleaned up and compliance issues remedied. He would send the well tender back to the site to complete the needed tasks.

Vortex Energy needs to: Empty the 100 BBL tank. Remove and properly dispose of the oil. Remove and properly dispose of all impacted leaf litter, grass, soil and debris under and around the two tanks. Repair surface area affected by the oil release. Build a dike around the two tanks.

0373090904 2014-03-17 Production Wells No Violations My inspection found the same production chart inside the separator since my previous inspection on 3/10/2014 with double markings for production. The valve on the flow line at the well was still open. I took additional photos.
0501665919 2019-04-23 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice # 1553259903, regarding the oil spill from the tank overflow. At the time of this inspection, free oil was still present between and under the 100 BBL and 50 BBL tanks. Oil impacted leaf debris and soil was still under and around the 50 BBL and 100 BBL tanks. The 100 BBL tank fluid level was still approximately 3 to 4 foot below the top of the tank. The 50 BBL tank fluid level was still approximately inches below the top of the tank and has not been emptied. No dike has been installed around the tank battery. The well was shut-in at the separator, at the time of this inspection. I contacted Vortex Energy, LLC and left a message with field superintendent, Greg Henderson regarding the cleanup that was still needed from the oil spill, removal of the impacted soil and proper disposal of the impacted material. The dike still needs installed around the tanks. The 100 BBL and 50 BBL tanks still need to be emptied.

0790161441 2017-10-17 Production Wells No Violations I was on location to assess compliance notice # -960462216. I found the well still surrounded by thorns. The well is in production with an open gate valve on the tubing. The overgrown vegetation around the two (2) storage tanks has been removed. Some clean--up of the tank battery has begun and he still needs to cut down the vegetation around the well. Photos were taken and are available upon request.
0821938125 2019-03-21 Production Wells Violations Noted I conducted a compliance notice follow-up inspection for compliance notice # 1553259903 issued on 3/20/2019, regarding the oil spill from the tank overflow. Upon arrival on the site, well tender Mason Barr from Vortex Energy was on location and a large fire was burning under and between the 100 BBL and 50 BBL tanks. Upon questioning Mr. Barr about the fire, he stated that he was trying to burn off the oil as a cleanup effort when the whole area caught fire. Mr. Barr did not have a way of putting out the fire. I contacted Hartford Fire Department and explained the current fire situation at this location. Hartford Fire Department dispatched a crew to put out the fire. Hartford Police Department officer Ptl. Theresa Hartman, was also present on site at the same time as the fire department. The fire department was able to put out the fire. A compliance notice has been issued for 1501:9-9-02 General Safety. The utilization of a fire as a means to clean-up free crude oil, present between production tanks containing product, is not a reasonable means to safeguard against hazards of life, limb and property. The 100 BBL tank and 50 BBL tank had not been emptied, prior to the fire being started and still needed to have the fluid removed. The area of cleanup where oil impact had occurred still needs to be cleaned up and any material disposed of properly. The well had been shut-in at the mechanical separator, at the time of this inspection.

1125962112 2014-03-10 Preliminary Restoration No Violations I was on location to perform a follow-up inspection to compliance notice #1178977238, which was issued in reference to Chiefs Order 2014-43. My inspection found the valve on the flow line at the well open. There were open valves leading to and exiting the separator. The production chart inside the separator was changed and had a date of 3/6. I took additional photos of the production chart and the well. The well was in production which was in non-compliance of Chiefs Order 2014-43. The well needs to be shut-in immediately.
1178977238 2014-03-05 Production Wells Violations Noted I was on location to check compliance to Chiefs Order 2014-43. My inspection found the well was equipped with a plunger lift unit. The valve leading to the flow line from the well was open. There was a domestic flow line coming off the 4.5 casing. The tank battery consisted of a 100 barrel production tank, a 50 barrel brine tank, and a heat, treated separator. The separator had the correct spacing. There was an identification sign posted on the separator with no identification. The dike was completely weathered and not meeting containment requirements. The flow line valves leading to and exiting the separator were open. There was a production chart inside the separator with no date. The lease road was in stable condition. Photos were taken of all the production equipment. The well was in production which was in non-compliance of Chiefs order #2014-43. The well needs to be shut-in immediately.
1240686677 2014-04-25 Production Wells No Violations I was on location to re-inspect in reference to Chiefs Order 2014-43 and Compliance Notice #1178977238. My inspection found the main ball valve at the 4 Concoyl Unit open. The ball valve on the flow line at the well was open. There was a production chart inside the separator with a date of 4/15 and no year. The dike was completely weathered and not capable of meeting containment requirements. I noticed oil stained soil around the brine tank and production tank. I took photos of the well, separator, production chart and tank battery. The well was in production which was in non-compliance of Chiefs Order 2014-43.
1394127941 2014-03-06 Production Wells No Violations My inspection was a follow-up to compliance notice #1178977238. I took additional pictures of the production chart inside the separator as evidence that the well was in production. The well was in production which was in non-compliance of Chiefs order #2014-43. The well needs to be shut-in immediately.
1398702807 2014-04-28 Production Wells No Violations I was on location to re-inspect in reference to Chiefs Order 2014-43 and Compliance Notice #1178977238. The same production chart from my inspection on 4/25/2014 was inside the separator; however, the movement of the chart showed production. The dike was completely weathered and not capable of meeting containment requirements. I noticed oil stained soil around the brine tank and production tank. I took photos of the well, separator, production chart and tank battery. The well was in production which was in non-compliance of Chiefs Order 2014-43.

1553259903 2019-03-20 Production Wells Violations Noted I conducted an inspection on this well location as a request from landowner James Tomory regarding oil on the ground from an overflowed production tank. There is an open compliance notice # 960462216 involving a previous oil release, written on 4/27/2016 for this well location. At the time of this inspection, the well was equipped with a plunger lift assembly and in production. The gate valve on the tubing and the 2 inch flow line valve were open. The tank battery consisted of one 100 BBL tank, one 50 BBL tank and a heater treater separator. Valves on the inlet and outlet lines on the separator were open. The chart on the the separator showed production. New, free oil was pooled under the 100 BBL and 50 BBL tank and had migrated approximately 40 feet by 4 foot north of the tank battery location. The side of the 100 BBL tank was shiny where the oil had overflowed and upon inspection of the tank, the fluid level was at the top of the tank. The 50 BBL tank was approximately 75% full of fluid. There was no dike surrounding the production tanks or separator. Legible identification with current owner information has been installed at the well and separator locations. I contacted Vortex Energy, LLC and spoke with field superintendent, Greg Henderson, regarding the oil spill and the need for immediate remediation of the site. I also stated that dike would need to be installed around the production tanks. Mr. Henderson stated that he would send his pumper out to the location immediately to empty the 100 BBL tank and begin the cleanup of the spilled oil, along with the removal of the impacted material. He also stated that a new dike would be installed.

Vortex Energy needs to: Empty the 100 BBL tank. Remove and properly dispose of the oil. Remove and properly dispose of all impacted leaf litter, grass, soil and debris under and around the two tanks. Repair surface area affected by the oil release. Build a dike around the two tanks.

1553519627 2019-03-21 Production Wells No Violations I conducted a follow-up inspection for compliance notice # 821938125 issued today on 3/21/2019, regarding a crude oil fire at the tank battery location. The fire had been extinguished by Harford Fire Department. Upon reinspection of the location, no fire was present and no cleanup efforts were underway. Compliance notice issued for general safety is closed.
1554906261 2019-04-10 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice # 1553259903, regarding the oil spill from the tank overflow. At the time of this inspection, free oil was still present between and under the 100 BBL and 50 BBL tanks. Within the oil impacted soil, salt crystals had formed near the 50 BBL tank. Oil impacted leaf debris was still under and around the 50 BBL and 100 BBL tanks. The 100 BBL tank fluid level had been dropped by approximately 3 to 4 foot below the top of the tank. The 50 BBL tank fluid level had increased and was approximately 3 foot below the top of the tank. The well was shut-in at the separator, at the time of this inspection. I contacted Vortex Energy, LLC and left a message with field superintendent, Greg Henderson regarding the cleanup that was still needed from the oil spill, removal of the impacted soil and proper disposal of the impacted material. The dike still needs installed around the tanks. The 100 BBL and 50 BBL tanks still need to be emptied.

At the time of this inspection, no one from Vortex Energy was on location, however some of the impacted soil north of the tank battery has been scraped up and removed. Free oil was still present between and under the 100 BBL and 50 BBL tanks. Debris under and around the 50 BBL and 100 BBL tanks was observed to have been again set on fire and the charred remnants were still smoking, at the time of this inspection. The 100 BBL tank was still full of fluid and needs emptied. The free oil and impacted soil needs removed from location and disposed of properly. I contacted Vortex Energy, LLC and left a message with field superintendent, Greg Henderson regarding the rest of the cleanup still needed from the oil spill and the dike still needs installed, along with emptying the 100 BBL tank.
