0365399809 2014-11-10 Production Wells No Violations I was on location for a status check. During my inspection, I found the well not in production. The well was equipped with a plunger lift assembly. The flow line ball valves were in the closed position. The well was supplying gas to a domestic line coming off of the 4.5 casing. The tank battery consisted of a 100 barrel production tank and a vertical separator. I noticed a small amount of oil and oil stained soil on the north side of the production tank. Over three quarters of the dike was completely weathered. An identification sign was properly posted. A bull plug was installed on the load line. The lease road was in stable condition. I researched our database and found zero production reported for 2013. I called and spoke with Bill Templeton. I instructed Mr. Templeton to:




Mr. Templeton stated he could not replace the production tank at this time, because of his mother. Mr. Templeton stated he was taking care of his mother; who was on hospice care. At this time, I told Mr. Templeton to:



Photos were taken. I informed Mr. Templeton follow-up inspections will be conducted until all issues are taken care of.

-up line has a plug. The access road is in stable condition and grass covered. I spoke with Bill Templeton, owner of Dustin Energy, to repair the gas leak at the wellhead. Mr. Templeton stated the well tender would be dispatched immediately to repair the gas leak. Also, Mr. Templeton stated he has been waiting on East Ohio Gas to repair their Positive Shut-off in order to place the well back into production.

Mr. Templeton stated he would contact his well tender to respond to the gas leak. Also, Mr. Templeton stated the well does supply domestic gas.

I spoke with Ms. Crawder on location. Ms. Crawder stated the well has been leaking for approximately one month. Also, Ms. Crawder claims the well has leaked off and on throughout the past winter.

I informed Ms. Crawder that I contacted Dustin Energy to immediately make repairs to stop the gas leak and a follow-up inspection will be conducted.

1499795689 2017-07-11 Production Wells No Violations I found the gas leak at the wellhead repaired per my request.
1530622070 2018-07-03 Production Wells No Violations I received a call from Cindy Crawder on 7/1/2018. Ms. Crawder stated the well on her property still had a gas leak. Also, Ms. Crawder stated Bill Templeton, owner of Dustin Energy, was on location attempting to repair the gas leak. Ms. Crawder claimed the lid on the storage tank blew open and oil could be see on the side of the tank.

I immediately contacted Mr. Templeton. Mr. Templeton stated he was on location to tighten the tubing wellhead. Also, Mr. Templeton stated the lid blew open at the storage tank from him blowing down the well. Mr. Templeton stated there was no oil leak or spill; that the storage tank was misting from blowing down the well. According to Mr. Templeton, he stated the gas leak has been stopped and he verified it by pouring water on top of the wellhead to ensure there were no bubbles.

I instructed Mr. Templeton to clean-up any oil residue from making repairs. Also, I informed Mr. Templeton that I would be on location early on Monday, 7/2/2018.


I contacted Mr. Templeton and told him the wellhead was still leaking and needs repaired. Also, I instructed Mr. Templeton to clean-

2042765846 2018-07-18 Production Wells No Violations I found the dried old oil at the base of the storage tank removed per my request. There is still a very minor gas leak at the wellhead. I spoke with the landowner, Cindy Crawder, on location.

I informed Ms. Crawder: The old oil has been cleaned up. Bill Templeton is waiting on a service rig to replace the rubbers at the wellhead.

2055241108 2018-07-11 Production Wells No Violations I found Dustin Energy has not removed the dried old oil at the base of the 100 bbl. storage tank per my request. There is still a minor gas leak from the tubing packing head. The gate valve on the tubing is in the closed position. The two (2) flow line ball valves are in the closed position. An identification sign is posted. I spoke with Bill Templeton, owner of Dustin Energy. Mr. Templeton stated his well tender will have the dried old oil removed by Tuesday, July 17, 2018 when he is in the area changing charts. Also, Mr. Templeton stated he is waiting on a swab rig to become available to change out the rubbers at the wellhead. An inspection will be conducted by July 18, 2018.
