0138034457 2014-06-24 UrbanProduction Wells No Violations I was on location to perform a final restoration. I found during my inspection that the 100 barrel production tank and separator were still in place. I called Woody McDaniel of Enervest and instructed him to:

Remove all of the production equipment Grade and seed the well site Work must be completed by 7/1/2014. Follow-up final restoration inspection will be conducted on that date. The lease road is shared with permit numbers 2-2963 and 2-2964.

0336589443 2014-07-28 Final Restoration No Violations I was on location for a final restoration. I found during my inspection all of the production equipment removed. The site was graded. Seed was spread. The pit was filled and closed. All debris was removed. The lease road was left in place to service existing wells. Final restoration passed.
0591581344 2014-03-19 Plug / Plug Back No Violations I witnessed Nicks Well Plugging set the third plug for Enervest. Plug #3 was set from 481 to surface. The Berea Sandstone was logged at 84 to 122. Circulation was established with 12 barrels of water. The operator mixed and pumped 146 sacks of Class A cement. I witnessed good cement returned to surface. The operator removed the final 11 joints of 4.5 casing. The well was topped off with cement and held static at surface. A total of 76 joints of 4.5 casing were removed from the well.
0859963589 2013-05-15 Administrative Inspection No Violations Enervest Operating filed and was issued a plugging permit on 5/8/2013 per my request from an inspection on 11/15/2012.
0926897392 2014-07-16 UrbanProduction Wells No Violations I was on location to meet with Lance, a KDA employee. KDA was contracted by Enervest to restore the site. I instructed Lance to:

Remove the production tank. Remove the separator. Grade and seed the site. Lance stated the work would take approximately one week. I will perform a final restoration once all of the tasks are complete.

0994961529 2012-11-15 UrbanProduction Wells Violations Noted I completed an urban production well inspection status check. My inspection found the well equipped with a plunger lift unit that was shut in. The entire tank battery and dike was overgrown weeds and contained one, 100 barrel oil/brine storage tank. The separator was outside the diked tank battery, the previous well owners identification sign was posted, and surrounded by overgrown weeds. The lease road was shared by permit numbers 2-2963 and 2-2964. I researched our database and found the last production was submitted by Ranger Resources in 2008 and Enervest Operating submitted zero production in 2010 and 2011. I called Larry Lower at 216-570-8708 and informed him I was issuing a compliance notice for non-production (Ohio Administrative Code 1509.062(A)(1)).
1395232571 2014-03-18 Plug / Plug Back No Violations I witnessed Nicks Well Plugging plug the well for Enervest. First, Nicks operator removed 4,320 of 1.5 of tubing. Then, Plug #1 was set from 4,048 to 4,494. The operator bullheaded the bottom plug by pumping 34 sacks of Class A cement with 50 lbs of Hulls. The cement was displaced with 64.3 barrels of water. The well was shut-in for 1 hour to allow the plug to set-up. Next, the operator ripped the 4.5 casing at 3,256 and 9 joints were pulled to set the Big Lime plug. Plug #2 was set from 2,570 to 2,870. Circulation was established with 40 barrels of water. The operator mixed and pumped 86 sacks of Class A cement. The cement was displaced with 40.8 barrels of water. Next, 56 joints of 4.5 casing was removed to set the Ohio Shale/Berea Sandstone plug. The well was shut-in overnight and the final plug would be set on 3/19/2014.
