The equipment on the site consists of the following:

1863819774 2013-02-06 UrbanProduction Wells No Violations My inspection found the well equipped with a plunger lift unit. It was surrounded by a locked chain link fence with the proper identification sign. The tank battery was surrounded by a locked chain link fence and the proper dike. It contained two, 210 barrel production tanks and two separators. There were plugs in the load lines and the proper identification sign was posted. The separators had 7 day production charts. The lease road was in good, stable condition. It had a locked gate at the entrance with a metal identification sign. This well shares a tank battery with permit number 2-3441. The production records were up to date on our database. This well was placed into production prior to House Bill 278.
1973794951 2013-06-05 UrbanProduction Wells No Violations I performed an inspection as a request from ODNR Hydrologist Kyle Tharp in reference to complaint ID 4344. My inspection found the well in production. The well was equipped with a plunger lift unit and surrounded by a locked chain link fence. The tank battery contained two, 210 barrel production tanks and two, separators. There were plugs on all of the load lines. The tank battery was surrounded by a locked chain link fence. The proper identification was posted at the tank battery and wellhead. There were 7 day production chart in the separators. I noticed no visible spills or leaks within the tank battery, wellhead, or surrounding area. The dike was in good condition and capable of meeting containment requirements. The lease road was in good, working condition with a locked gate at the entrance. The tank battery was shared with permit number 2-3441. This well was placed into production prior to House Bill 278.
