0220745428 2013-03-05 UrbanPreliminary Restoration No Violations My inspection found the well site graded and vegetation growing. The pits were filled and debris removed. The well was equipped with a plunger lift unit. It was surrounded by a locked chain link fence with the proper identification sign. The tank battery contained one, 210 barrel production tank, one, 50 barrel brine tank, and a separator. It was surrounded by a locked chain link fence with the proper identification sign. There was a plug on the load line. The required urban safety equipment was installed on the production tank. The dike was weathered and not capable of meeting containment requirements. The well was in production with a 7 day chart inside the separator. The lease road was in good condition. I called the office of PAC Drilling at 330-874-3781. I instructed them to repair the dike to meet containment requirements.

0931583354 2019-04-09 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice # 1900850142, regarding multiple compliance issues related to urban rule requirements. At the time of this inspection, the tank valve and pick-up lines still need locks installed or the handles removed. The identification posted at the tank battery is missing the required information per 1501:9-9-05(E)(1)(a-e). A locked chain link fence with barb wire surrounds the tank battery. The 210 BBL tank extends more than 3 foot above the fencing. No landscaping has been installed around the well or tank battery. Identification still needs posted on the access road gate.

1677755965 2012-10-25 UrbanPreliminary Restoration No Violations I conducted an Urban Preliminary Restoration inspection. My inspection found the well equipped with a plunger lift unit surrounded by a chain link fence. The tank battery was surrounded by a chain link fence, the proper dike, and contained one, 210 barrel oil storage tank with no carbon filter, lightning arrestor, and low pressure relief valve, one, 50 barrel brine storage tank, a separator, and the proper identification sign. I noticed no visible spills or leaks. The lease road was in good condition. I called office for PAC Drilling at 330-874-3781 and request the missing equipment for the storage tank be installed immediately to meet the Ohio Revised Code (ORC) 1501:9-9-5 under urban requirements. I will re-inspect by 11/25/2012.

1900850142 2019-03-05 UrbanProduction Wells Violations Noted I conducted an inspection on this well. At the time of this inspection, the well was equipped with a plunger lift assembly and in production. The gate valve on the tubing was open, and the 2 inch flow line ball valve coming off of the well was open. There was an identification posted at the well. There was a locked chain link fence around the well. The diked tank battery has one 210 production tank, one 50 BBL tank and a mechanical separator. Valves on the separator were open. The tank valve and pick-up lines have plugs, but are missing locks. The 210 BBL tank has the required urban safety equipment. There was identification posted at the tank battery, but is missing the required information per 1501:9-9-05(E)(1)(a-e). A locked chain link fence with barb wire surrounds the tank battery. The 210 BBL tank extends more than 3 foot above the fencing. There was no landscaping around the well and tank battery; however, they are located in a clearing surrounded by trees and vegetation. The access was stable and had a locked gate at the entrance. No identification sign posted on the access road gate.
