0038568781 2019-02-27 UrbanProduction Wells No Violations I conducted a compliance notice follow-up for compliance notice 2074402581, regarding multiple compliance issues related to urban rule requirements. At the time of this inspection, The pick-up lines have plugs, and handles removed at the tank battery location. Identification posted at the tank battery has now been amended with the required information per 1501:9-9-05(E)(1)(a-e). There was a chain link fence around the tank battery but was not eight feet in height and needed a lock on the man door. The 210 BBL storage tank extends more than 3 foot above the top of the fencing. The access road was stable, but no access gate had been installed. The access road gate was not waived during the pre-site inspection.

PAC needs to: 1) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05(E)(4)(f). 2) Install the required gate with a lock at the access road entrance per 1501:9-9-05(E)(2)(c). 3) Post the proper identification sign on the access road gate per 1501:9-9-05(E)(1)(f).

0882994755 2009-10-27 Preliminary Restoration No Violations Check well per complaint from Gary Litch [ TWP Trustee ] , Truck traffic late at night disturbing local residents . I contacted Jason Caldwell and advised him of complaint per truck traffic at wells late at night , I also advised him in urbanised areas maintenance or service on wells are to occur only between 7 AM and 7 PM 7 days a week unless it is an emergency . He advised me he would check into it and stop traffic to well in resticted hours . No violations at this time .

-up lines have plugs, but no locks. Both of the 210 bbl. storage tanks have the required urban safety equipment. There is an identification sign posted at the tank battery. The identification sign is missing the required information per 1501:9-9-05(E)(1)(a--site inspection. The tank battery is shared with permits #2-3887 and #2-3890.

The equipment on the pad consists of the following:



2074402581 2018-05-16 UrbanProduction Wells Violations Noted At the time of this inspection, the well was equipped with a plunger lift assembly. The annulus and flow line valves were open. The well was fenced with a locked door and had legible signage. The tank battery has three storage tanks and a separator. The pick-up lines have plugs, but no locks. Both of the 210 BBL storage tanks have the required urban safety equipment. There was an identification sign posted at the tank battery, but was missing the required information per 1501:9-9-05(E)(1)(a-e). There was a chain link fence around the tank battery but was not eight feet in height and needed a lock on the man door. PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05(E)(4)(f). 3) Install the required gate with a lock at the access road entrance per 1501:9-9-05(E)(2)(c). 4) Post the proper identification sign on the access road gate per 1501:9-9-05(E)(1)(f).
