The equipment on the site consists of the following:



On 12/13/2016, Northeast Regional Supervisor Tom Hill and I met with Justin Caldwell, co-owner of PAC Drilling, and Rusty Hornhill, well tender for PAC Drilling concerning the missing required urban safety equipment on 31 PAC Drilling wells in Hubbard and Liberty Townships. Mr. Caldwell has been given a document listing the wells with the missing the urban safety equipment. PAC Drilling is to submit a schedule for equipment installation for review and approval by the Division of Oil & Gas.

0507043898 2017-04-24 UrbanProduction Wells No Violations I found the well in production on the day of my inspection. The well is equipped with a plunger lift assembly. The gate valve on the tubing is open. The identification sign is posted at the well. The fencing around the well was not waived during the pre---up line has a plug, but no lock. The 210 bbl. storage tank is missing the required urban safety equipment. There is a deduct meter and 31 day chart dated 3/27/17 at the separator. The identification sign is missing the required information per 1501:9-9-05(E)(1)(a--site inspection.

The equipment on the pad consists of the following:



0679393225 2018-05-14 UrbanProduction Wells Violations Noted At the time of this inspection, the well was equipped with a plunger lift assembly. The annulus and flow line valves were open. The identification sign was posted at the well. The fencing around the well has not been installed and was not waived during the pre-site inspection. However, the well was located in the middle of a large open field surrounded by woods and approximately 750 feet from the nearest private residence. The diked tank battery has two storage tanks and a separator. The pick-up line has a plug and the valve handle has been removed. The 210 BBL storage tank was missing the required urban safety equipment. It was observed that small holes were starting to form near the top of the 210 BBL tank. The identification sign was missing the required information per 1501:9-9-05(E)(1)(a-e). A locked chain link fence surrounds the tank battery. The production tank was over 3 foot above the top of the fence. The access was stable, with a locked gate between the tank battery and access to the well. Landscaping was not waived during the pre-site inspection and still needs installed.

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Post the proper identification sign on the access road gate per 1501:9-9-05(E)(1)(f). 3) Complete the required landscaping around the tank battery per 1501:9-9-05(E)(6)(a-c). 4) Install the fencing as required around the well with a lock per 1501:9-9-05(E)(2)(a-d). 5) Install urban safety equipment on the production tank as required by 1501:9-9-05(E)(4)(b-e): Activated charcoal filters. Functioning low-pressure relief valve. Tank hatch lids shall have a functioning seal and the hatch shall be secured at all times. Functioning lightning arrestor. Remove the 210 BBL with holes on side.

0714095141 2019-02-04 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 679393225, regarding multiple compliance issues related to urban rule requirements. At the time of this inspection, the fencing around the well still has not been installed and was not waived during the pre-site inspection. The 210 BBL production tank now has the required urban safety equipment installed. The identification sign located at the tank battery was amended with the required information per 1501:9-9-05(E)(1)(a-e). A locked chain link fence surrounds the tank battery. The production tank was over 3 foot above the top of the fence. The access had a locked gate between the tank battery and access to the well. Required identification still needs installed on the lease access gate. Landscaping was not waived during the pre-site inspection and still needs installed.

PAC needs to: 1) Post the proper identification sign on the access road gate per 1501:9-9-05(E)(1)(f). 2) Install the fencing around the well. 3) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05(E)(4)(f). 5) Install the landscaping at the tank battery location.
