The equipment on the site consists of the following:



1542757539 2019-03-13 Production Wells Violations Noted I conducted an inspection on this well. At the time of my inspection, the well was equipped with a 4.5 inch casing and ball valve. The valve on the casing was closed and the 2 inch flow line has been disconnected at the well location. There was a 2 inch line that was plumbed above grade from the 8 5/8 inch casing. A ball valve connection was plumbed onto the 2 inch line. Brine was observed leaking from a loose fitting on the the valve connection. Staining on the wooden base behind the well from the leak was also present, at the time of this inspection. The production records were submitted in 2016 with 32 BBL of oil and 100 mcf of gas. In 2017, production records were submitted as zero. The tank battery has three 100 BBL tanks, two 50 BBL tanks and two separators. A Dominion East Ohio master meter was located within the diked battery. A compressor was also present adjacent to the tank battery on the west side. Identification was posted at the tank battery and well. The load lines are secure with locks and plugs. The production equipment and access road are in good repair. The well has its own production equipment but shares the diked tank battery location with permit number 2-4074. I contacted Cedar Valley Energy, Inc. and left message with the main office to discuss the leak and future plans for this well. The leak at the well needs to be remedied immediately and the well put back into production or plugged.

1822799454 2012-09-14 Production Wells No Violations I completed a production preliminary restoration inspection. My inspection found the well equipped with a pumping unit that was in good condition. The tank battery contained one, 100 barrel oil storage tank, one, 50 barrel brine storage tank, a separator, and the proper identification sign. The production equipment had the proper spacing, all debris has been removed, the proper dike has been installed, and all pits have been closed. The lease road was in good condition.
