The well was found without identification and a completion report on file. Tank battery does not have fencing or required equipment.

0460166571 2018-05-11 UrbanProduction Wells Violations Noted At the time of this inspection, the well was equipped with a plunger lift assembly. The annulus and flow line valves were open. A domestic gas supply comes off of the wellhead with an open 2 inch flow line with a regulator, drip tank and meter. There was no identification sign posted at the well. There was no fencing around the well. The fencing was not waived during the pre-site inspection. The diked tank battery has two storage tanks and a separator. The top of the production tank extends more than 3 foot above the fence. Tree damage occurred to the tank battery fencing and the 50 BBL poly tank top was crushed. The chain link fence needs to be repaired from the tree damage and the 3 strand barb needs to be added to the fencing. The pick-up lines have plugs and handles removed. An identification sign needs to be reattached to the chain link fence. The identification sign is the missing required information per 1501:9-9--e). There is no landscaping at the tank battery; however, the tank battery was surrounded by vegetation and trees. The access road was stable. The access road gate was waived on the pre-

0861702149 2012-12-31 UrbanProduction Wells No Violations I investigated a compliant by Mrs. OMalia about white smoke coming off this well. My inspection found no leaks or spills at the tank battery. The proper safety equipment was installed on the production tank. The separator was in good operating condition. The chain link fence was lock and the proper identification was posted.
0902393550 2012-08-06 UrbanProduction Wells No Violations I investigated a complaint regarding dead grass along Sodom-Hutchings Road near a P.A.C. Drilling well. My inspection found dead grass approximately 20 long by 3 wide surrounding a valve box with a gas valve. I called Jason Caldwell at 330-340-6238 and found that his well tender sprayed weed killer to keep the valve box exposed in case of an emergency.
1344277125 2012-08-06 UrbanProduction Wells No Violations On 8/6/2012, I completed an urban production well inspection status check. My inspection found the well was equipped with a plunger lift unit (rabbit) that was in good condition, newly painted and the proper identification was posted. The tank battery was surrounded by a chain link fence and contained one 210 barrel oil storage tank, one 50 barrel brine storage tank, a separator surround and the proper identification was posted. I noticed no visible spills or leaks. The lease road was in good condition. There are no violations at this time.
1347286299 2012-08-06 UrbanPreliminary Restoration No Violations I completed an Urban Preliminary Restoration inspection. My inspection found the well equipped with a plunger lift unit (rabbit) with the proper identification posted. The tank battery was surrounded by a chain link fence with a lock and contained one 210 barrel oil storage tank, one 50 barrel brine storage tank, a separator, and the proper identification. The production equipment had the proper spacing, all the debris had been removed, and the pits had been filled. The site was graded and seed to prevent erosion and sedimentation. The lease road was in good condition. Inspection passed.
1480730351 2017-08-21 UrbanProduction Wells No Violations I found the tank battery damaged by fallen trees caused by a tornado that went through the area on August 17, 2017. The chain link fencing surrounding the tank battery, top of the 50 bbl. poly storage tank, and five (5) gallon charcoal filter have been damaged by the trees. The integrity of the two (2) storage tanks has not been affected. The wellhead was not impacted by the tornado. I spoke with Rusty Thornhill, PAC well tender. Mr. Thornhill stated he is working on clearing the debris and then will make all necessary repairs. Follow-up inspections will continue and the tank battery has been repaired.

At the time of this inspection, no changes have occurred since my last inspection. The fencing and signage at the well location has not been installed. The fencing around the diked tank battery has not been repaired from the tree damage. The 50 BBL poly tank still needs to be removed due to damage from the tree debris. The identification sign needs to be reattached to the chain link fence and added information per 1501:9-9--e), still needs completed.
