0214699052 2018-05-21 UrbanProduction Wells Violations Noted At the time of this inspection, the well was equipped with a plunger lift assembly, and the annulus and flow line valves were open. There was an identification posted on the well. There was no fencing around the well. The fencing was not waived during the pre-site inspection. The diked tank battery has four storage tanks and two separators. The pick-up lines have plugs and the handles on the valves have been removed. The required urban safety equipment has been installed on the production tanks. There was a locked chain link fence around the tank battery. The top of the production tanks extend more than 3 foot above the fence. An identification sign was posted on the fencing, but was missing the required information per 1501:9-9-05(E)(1)(a-e). There was no landscaping at the tank battery; however, the tank battery was located in a clearing approximately 700 feet from Route 7 and hidden behind a large commercial warehouse. The access road was stable with no gate at the entrance. The access road currently shares the entrance for a commercial warehouse and was not required during the pre-site inspection. The tank battery was shared with permit #2-4010.

1343660349 2012-07-30 UrbanProduction Wells Violations Noted On 7/30/2012, I conducted an urban production well inspection status check. My inspection found the well was a plunger lift unit (rabbit) that was in good condition, but with no identification. The tank battery was shared with permit number 2-4010 and contained two 210 barrel oil storage tanks, two 50 barrel brine storage tanks, two separator, and no identification. The lease road was in good condition and shared with permit number 2-4010. I called Jason Caldwell of PAC Drilling at 330-340-6238 and asked for the tank battery and well to be proper identified by 8/10/2012.
1447661360 2019-03-08 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 214699052, regarding multiple compliance issues related to urban rule requirements. At the time of this inspection, there was still no fencing around the well. The fencing was not waived during the pre-site inspection. The identification posted at the tank battery has been amended with the required information per 1501:9-9-05(E)(1)(a-e). The 210 BBL tank is taller than the tank battery fence. There was no landscaping at the tank battery; however, the tank battery was located in a clearing approximately 700 feet from Route 7 and hidden behind a large commercial warehouse.

PAC needs to: 1) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05(E)(4)(f). 2) Install the fencing around the well.
