0768427995 2019-03-01 UrbanProduction Wells No Violations I conducted a compliance notice follow-up for compliance notice 1984658839, regarding multiple compliance issues related to urban rule requirements. At the time of this inspection, the required fencing has not been installed around the well. The fencing was not waived during the pre-site inspection. The 210 BBL storage tank was missing the required urban safety equipment, except for the charcoal filter. The identification sign has been filled in with owner information but still needs amended with required information per 1501:9-9-05E(1)(a-e). The production tank is over 3 foot above the top of the fence. There was no landscaping at the tank battery and well. There was no access gate installed.

PAC needs to: 1) Post an identification sign to be in compliance with 1501:9-9-05E(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Install urban safety equipment on the production tank as required by 1501:9-9-05E(4)(b-e): Functioning low-pressure relief valve. Tank hatch lids shall have a functioning seal and the hatch shall be secured at all times. Functioning lightning arrestor. 3) Install a fence around the wellhead as required by 1501:9-9-05E(2) 4) Place landscaping on the west and south sides of the tank battery as required by 1501:9-9-05E(6). 5) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05E(4)(f). 6) Install the required access gate with a lock at the access road entrance per 1501:9-9-05(E)(2)(c).

The equipment on the pad consists of the following:



1536628691 2018-05-17 UrbanProduction Wells No Violations I conducted a compliance notice follow-up for compliance notice 1984658839. The well was equipped with a plunger lift assembly and the annulus and flow line valves were open. A domestic gas supply comes off of the well. An identification sign was posted at the well but no fencing has been installed around the well. The fencing was not waived during the pre-site inspection. The diked tank battery has two storage tanks and a separator. The pick-up lines have plugs and the ball valve handles have been removed. The 210 BBL storage tank was missing the required urban safety equipment. The identification sign posted at the tank battery was faded and unreadable. There was a locked chain link fence with barb wire surrounding the tank battery. The production tank is over 3 foot above the top of the fence. The tank battery and well are located within 70 feet of Drummond Ave. There was no landscaping at the tank battery and well. The access road was stable. There was no access gate installed.
