0025208600 2015-09-04 Production Wells No Violations I was on location for a status check. During my inspection, I found the well in production. The well is equipped with a plunger lift assembly. The tank battery consists of a 100 barrel production tank, a 50 barrel, poly brine tank, and a mechanical separator. There is a dike around the tank battery. An identification sign is posted. A plug is installed on the load line. The lease road is in stable condition.

1555003422 2019-04-11 Production Wells No Violations I conducted a status check on this well. The well was equipped with a plunger lift assembly and in production, at the time of this inspection. The gate valve on the tubing and the 2 inch flow line valve were open. There was a domestic line plumbed off the 4.5 inch casing with two regulators, a drip bottle and meter. The valve on the domestic line was open. The well was surrounded by a chain link enclosure. The tank battery was comprised of one 140 BBL tank, one 50 BBL tank and a mechanical separator. Also present within the diked tank battery was a Dominion East Ohio master meter. Valves on the separator were open and the chart showed production. The tank battery was surrounded by a chain link enclosure. Legible identification was present at the tank battery. The lease access was stable.

The well was a plunger lift unit. The tank battery consisted of a 100 barrel production tank, a 50 barrel brine tank, and a separator. A dike surrounded the tank battery. Locked, chain link fences surrounded the production equipment and well. An identification sign was properly posted. A plug was installed on the load line. The lease road was in good repair.
