0295458299 2019-03-05 UrbanProduction Wells Violations Noted I conducted an inspection on this well. At the time of this inspection, the well was equipped with a plunger lift assembly and in production. The gate valve on the tubing was open. The 2 inch ball valve on the flow line coming off of the tubing was open. An identification sign was hanging from the 2 inch flow line. The diked tank battery has one 210 BBL tank, one 50 BBL tank and a mechanical separator. Valves on the separator were open. The load lines and tank valve have plugs and handles removed. The required urban safety equipment is installed on the production tank. An identification sign was posted at the tank battery, but still needed the required information added to it per 1501:9-9-05E(1)(a-e). There was no fencing around the tank battery and well. According to the pre-site inspection report dated January 13, 2011, the fence around the well and tank battery was not waived. The access road gate was waived. The leaes access was stable.

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05E(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Install the fencing around the well and tank battery per 1501:9-9-05E(2)(a-d).

1579981768 2012-12-07 UrbanPreliminary Restoration No Violations I conducted an urban preliminary restoration inspection. My inspection found the well equipped with a plunger lift unit with the proper identification sign. The tank battery contained one, 210 barrel oil storage tank, one, 50 barrel brine storage tank, a separator, and the proper identification sign. I noticed a ditch through the southeast corner of the dike. I called Jason Caldwell at 330-340-6238. I instructed Jason to have his well tender repair the dike immediately and that I would re-inspect by 12/11/12. The pits were closed and debris removed. The site was graded and vegetated.
1728601323 2017-08-21 UrbanProduction Wells No Violations I found that a tree limb has fallen across the diked tank battery missing all of the production equipment. The identification sign is lying on the ground. I spoke with Rusty Thornhill, PAC well tender. I told Mr. Thornhill to repost the identification sign and remove the tree limb by 8/25/2017.
