Drainage and erosion controls were in place and working properly. Grass was well established on the topsoil stockpiles and surrounding area. The proper identification was posted at the entrance. The site was surrounded by post and wire fencing with a locked gate at the entrance.

1121477726 2013-07-09 Production Wells No Violations My inspection found the tank battery complete. The tank battery contained four, 410 bbl tanks; two for produced water and two for crude oil. The dike was made of steel that contained a liner and pea gravel. Overall completion of the well was still in progress, technicians were on location working installing flow lines and production equipment. The proper identification was posted at the entrance. Drainage control were working proper. Vegetation was well established on the topsoil stockpiles surrounding the well pad.

1348357384 2019-03-21 Production Wells No Violations I conducted an inspection on this well site. At the time of this inspection, the well was equipped with a plunger lift assembly and the valves on the casing and flow line were open. The tank battery consisted of four 400 BBL production tanks and was diked. Other production equipment on site consisted of one butane tank and a separator. The location was stable and secondary earthern berm was intact. The signage was located on the locked gate that surrounded the site. The lease access was stable. No violations observed.

1357662409 2013-01-05 Drill / Deepen / Reopen No Violations I witnessed the cementing of the surface casing by Halliburton for Halcon Operating Company, Inc. Hole #2 was 26 in diameter and drilled to a depth of 405. The surface casing was 20 and set to a depth of 394. The surface casing assembly included a Halliburton 20 float shoe and a Halliburton Super Seal II Float collar (stab in) with one, 20 joint in between. Halcon used a Halliburton sealing sleeve adapter on the end of the drill pipe to stab into the float collar to run the cement. Halliburton filled the lines with 5 barrels of fresh water, broke circulation with 10 barrels of fresh water, pumped 50 barrels of fresh water plus 6% gel followed by 138 barrels of Class A cement. Six barrels of fresh water was used to displace the cement. I witnessed good cement return to surface. The cement held steady at the surface for 20 minutes. A wiper ball was used to displace cement inside the drill pipe leaving 40 of cement in the surface casing.

1359663100 2013-01-31 Drill / Deepen / Reopen No Violations I arrived at the site at 1445. The drillers have reached long string total depth (TD) of 10,929 (ground level). Production string is comprised of 5.5-inch casing in 8.75-inch diameter borehole. A total of 162 centralizers were deployed. Circulation of hole is in progress. Circulation from 1330 to 1500 hours. 2 times bottoms-up hole volumes achieved. Crews are commencing rig-up of cement drive head unit (Blackhawk top-drive cement head) . Company man (Brian Dutton) reports no issues or problems. At approximately 1700, cement drive head rigged up and circulation established. At 1800: safety meeting conducted by cementers (Haliburton) ahead of cementing operations. At 2145, cement circulating to the surface.
1403112802 2014-06-18 Preliminary Restoration No Violations I was on location for a preliminary restoration inspection. I found during my inspection surrounded by a fence with a locked gate. An identification sign was posted at the entrance to the well site. The berms on the south and west sides of the pad were vegetated. The land surrounding the well pad was for agriculture use. All drilling equipment had been removed. All debris was removed. Drainage and erosion controls were in place and working properly. The well was equipped for direct flow that was in production. The production tanks were surrounded by a lined steel dike. All of the production tanks spacing was in compliance. The well pad was in stable condition. The lease road was in excellent condition with crushed limestone. Restoration passed.
