0047196488 2014-05-06 Preliminary Restoration No Violations I was on location to perform a preliminary restoration inspection. I found during my inspection the well was in production. The well was direct flow. The tank battery consisted of three, 400 barrel production tanks, a compressor, and separators. A steel and lined dike surrounded the tanks. A flare was burning off flash gas from the production tanks. An identification sign was posted at the entrance to the well pad. Warning and safety signs were properly posted. Safety equipment was installed on the tanks. The lease road was in good repair. There was a locked gate at the entrance to the well pad. Vegetation was established on the berms. All drainage and erosion controls were in place.

Halcon ran 4.5 casing from 5,255 to 14,562, then 5.5 casing from 5,255 to surface. The 4.5 casing had an average length of 41, grading of P-110, and 15.1 lbs/ft. The 5.5 casing had an average length of 44, grading of P-110, and 22.5 lbs/ft. Halliburton established circulation. The operator pumped 100 bbls of Tuned Spacer III followed by 215 sacks of Gasstop System. Then, 1,170 sacks of Halcem was pumped. The operator displaced with 248 bbls. Cement was brought up to approximately 2,400 below ground level.

0494021946 2014-12-08 Preliminary Restoration No Violations I was on location for a preliminary restoration. During my inspection, I found the site graded and the berms around the well pad vegetated. No pits were used during the construction of the well; closed loop system. All drilling equipment was removed. All debris was removed. There was a locked gate at the entrance to the well pad. A wooden post and wire fence surrounded the well pad. The proper identification sign was posted. Erosion and drainage controls were in place. The tank battery consisted of two, 400 barrel tanks labelled for water and a 400 barrel tank labelled for oil. A line steel dike containing pea gravel surrounded all of the tanks. All of the load lines were capped and locked. The well was equipped with a plunger lift assembly. The flare was in operation burning off flash gas. Preliminary restoration passed.

0646939496 2014-06-03 Production Wells No Violations I was on location for a status check inspection. I found during my inspection the well in production. There were jersey barriers surrounding the well. Flash gas was being burned off at the flare. A dike surrounded the tank battery. A proper identification sign was posted at the entrance to the well pad. Proper drainage and erosion controls were in place and operating effectively.
0838791422 2013-05-29 Drill / Deepen / Reopen No Violations I witnessed Halliburton cement the conductor casing for Halcon. Halcon requested shorting Hole #1 from the permitted 90. They encountered hard sandstone at 40. I reviewed area wells and geologic formations, then told Halcon to drill the conductor hole to a minimum depth of 55. Rocky Mountain Drilling Augur rig SR-30 drilled a 36 Hole #1 to 57 and set 26 casing at 57. The conductor was rated at 163 lbs./ft. and 42 long; cut to length. Halliburton rigged up to the conductor and broke circulation with 10 barrels of fresh water. The operator pumped 20 barrels of gel spacer, followed by 355 sacks of Class A cement. Halliburton displaced the cement to 37 with 26 barrels of fresh water. I witnessed approximately 28 barrels of good cement returned to surface. Cement was sampled and weighed three separator times; at the beginning, middle, and end of the operation. Cement was inspected 20 minutes after completion and was static at surface.

1184581885 2013-06-06 Drill / Deepen / Reopen No Violations I witnessed Halliburton cement the surface casing for Halcon. Patterson Rig 256 drilled a 22 Hole #2 to 565. The operator set the 16 surface casing to 550. The casing had a rating of J-55; 65 lbs./ft. and 46 joints. Halliburton broke circulation with 35 barrels of fresh water. The operator pumped 50 barrels of 6% gel spacer, followed by 960 sacks Class A cement. Halliburton displaced the cement to 510 with 122 barrels of fresh water. I witnessed approximately 85 barrels of good cement returned to surface. Three samples were weighed to ensure the cement had the proper density.

According the field supervisor for Halcon, Justin, filling the impoundment will take approximately 32 days. Construction of the well pad is complete. All drainage controls are in place and working properly. Seeding and straw matting has been spread over all of the topsoil stockpiles.

1372344656 2013-06-15 Drill / Deepen / Reopen No Violations I witnessed Halliburton cement the 2nd Intermediate Casing for Halcon using the 2 stage method. Patterson Rig 256 drilled a 9.875 Hole #4 to 5,375 from RKB (Kelly). The operator set the 7.625 casing to 5,337 from RKB. The DV tool was set at 4,101. The casing had a grade of N-80 BTC, weight of 26.4 lb./ft., and an average length of 42.5. Stage 1 started with Halliburton establishing circulation with 5 bbls of fresh water. The operator pumped 39 bbls of Tuned Spacer III followed by 310 sacks of Halcem System cement. Halliburton dropped the plug and displaced with 240 bbls of salt brine. The bomb was dropped and free fell for 28 minutes. The operator pressured up and the DV tool opened at 550 psi. Next, Halcon circulated through the DV tool for 3.5 hours to allow the cement from Stage 1 to set up. Stage 2 began with Halliburton establishing circulation with 5 bbls of fresh water. The operator pumped 48 bbls of Tuned Spacer III followed by 650 sacks of Varicem Cement. Halliburton dropped the plug on the fly and displaced with 182 bbls of salt brine. The operator bumped the plug and closed the DV tool at 1500 psi over pressure that held for 5 minutes. I witnessed 1 bbl of spacer returned to surface.

1408772208 2018-01-04 Production Wells No Violations I conducted an inspection on this wellsite with DOGRM inspector, John Fleming. During this inspection, the well was equipped with a plunger lift assembly and the annulus and flow line valves were closed. Production records for this well were current. The tank battery consisted of three 400 BBL production tanks and was diked. Other production equipment on site included a portable compressor and a butane tank. No production equipment was operational at the time of this inspection. The site was stable and the signage was located at the gate. The lease access was stable. No violations observed.
1440511823 2015-08-07 Production Wells No Violations I was on location for a status check. During my inspection, I found the well not in production. The well is equipped with a plunger lift assembly. I observed the flow line valve at the well is in the closed position. The tank battery consists of three (3), 400 barrel production tanks and a condensate tank. The tanks are surrounds by a lined, steel dike. The load lines are secure with locks. An identification sign is posted at the entrance. The well site is surrounded by fencing. The lease road and well pad are is stable condition.
1522675323 2013-06-08 Drill / Deepen / Reopen No Violations I witnessed Halliburton cement the intermediate casing for Halcon. Patterson Rig 256 drilled a 14.75 Hole #3 to 1,523 RBK (Kelly). The operator set the 10.75 casing to 1,513 RBK. The casing had a grade of J-55; 40.5 lb/ft and 44.5 lengths. Halcon used 12 centralizers. Halliburton broke circulation with 5 barrels of fresh water. The operator pumped 50 barrels of gel spacer, followed by 830 barrels of class A cement. Halliburton displaced the cement to 1,455 using 137 barrels of fresh water leaving 45 of cement inside the casing. I witnessed approximately 20 barrels of good cement returned to surface. A cement sample was weighed to ensure proper density. The fresh water had a pH of 6 and a temperature of 65 degrees.

At the time of this inspection, Luke Mallett from Pin Oak Energy Partners was on location performing well and flow line maintenance. The well was not in production, during this inspection and was equipped with a plunger lift assembly. The tank battery consisted of three 400 BBL production tanks and was diked. Other production equipment on site included a portable compressor and a butane tank. Due to operational mainteance the production equipment was not operational at the time of this inspection. The site was stable. Identification was located at the locked gate. The lease access was stable.

I called and spoke with Zane Oshurac from Halcon. I instructed Mr. Oshurac to secure all man doors with locks and post an identification sign at the well.
