2014-01-30 ACID 0 500 Gal 50000 Universal Well Services Inc. On 1/30/2014, I witnessed Universal Well Service stimulate the Clinton Sandstone formation for PAC Drilling. The backside annulus was completely open during the stimulation operations with no failures. There were no casing failures. There was a second breakdown pressure of 1970 psi. A total of 30 balls were dropped. A total of 126,728 gallons of water was used. All of the stimulation equipment operated smoothly for Universal Well Service.

There is no fencing around the well; however, the well is located in a clearing surrounded by thick woodlands. The fencing was waived during the pre--up lines are secure with handles removed from the valves and plugs. The 210 bbl. storage tanks have the required urban safety equipment. There is an identification sign posted at the tank battery. The identification sign is missing the required information per 1501:9-9--e). The fencing around the tank battery was waived during the pre--4085. There is no identification sign on the gate.

PAC needs to: 1) Post an identification sign to be in compliance with 1501:9-9--e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 4) Post the proper identification sign on the access road gate per 1501:9-9- 3) Post an identification sign at the well per 1501:9-9-

0751449224 2018-05-15 UrbanProduction Wells No Violations I conducted an compliance notice follow-up inspection for compliance notice 243331845. The well was equipped with a plunger lift assembly and the annulus and flow line valves were open. There was still no identification posted at the well. No fencing surrounded the well; however, the fencing was waived during the pre-site inspection. The diked tank battery has three storage tanks and a separator. The pick-up lines are secure with handles removed from the valves and plugs. The 210 BBL storage tanks have the required urban safety equipment. There was an identification sign posted at the tank battery, but was missing the required information per 1501:9-9--e). The fencing around the tank battery was waived during the pre-site inspection. The access road was stable with two locked gates at the entrance and shared with permit #2-4085. Identification was needed on the gate.

--up lines have plugs, but no locks. The 210 bbl. storage tanks have the required urban safety equipment. A deduct meter and 31 day chart dated 1 March 2017 is installed at the separator. There is an identification sign posted at the tank battery. The identification sign is missing the required information per 1501:9-9-05(E)(1)(a-e). There is no danger, keep out and no smoking sign(s) posted. The fencing around the tank battery was waived during the pre--4085. There is no lock or identification sign on the gate.

The equipment on the pad consists of the following:



1390837587 2014-01-16 Drill / Deepen / Reopen No Violations I witnessed Universal Well Service cement the surface casing for PAC Drilling. Poulson Drilling drilled a 11 Hole #2 to 787 RBK (rig floor) and set 8.625 casing to 770 RBK with 2 centralizers. The casing was 23 lbs per ft. The bottom of the Berea Sandstone was logged at 632. Universal established circulation with 2 barrels of fresh water, then ran 12 barrel of sweep and 5 barrels of a fresh water spacer. Next, the operator pumped 150 sacks of Unifill Lite as the lead cement followed by 125 sacks of Type 1 as the tail cement. The cement was displaced to 750 with 48 barrels of fresh water. After 20 minutes, the cement held static at surface within the annulus of the casing.
1407159042 2014-08-04 UrbanPreliminary Restoration No Violations I was on location for an urban preliminary restoration. I found during my inspection the well site graded. The site was well vegetated. The pits were filled and closed. All debris was removed. Erosion and drainage controls are in place. The well was in production with a plunger lift unit. The tank battery consisted of two, 210 barrel production tanks, a 50 barrel brine tank and a separator. A dike surrounded the production equipment. The production equipment had the correct spacing. The proper identification was posted. Plugs were installed on the load lines. Urban safety equipment was installed on both production tanks. The lease road was in good condition. The tank battery was located in an isolated and wooded area; fencing was waived on the pre-site inspection. Urban preliminary restoration passed.
1427181320 2014-01-14 Drill / Deepen / Reopen No Violations I witnessed Universal Well Service cement the conductor casing for PAC Drilling. Poulson Drilling drilled a 14.75 Hole #1 to 93 RBK (rig floor) and set 11.75 casing to 84 RBK. The rig floor was 5 above ground level. The casing had a weight of 32 pounds per foot. The Universal operator established circulation with 3 barrels of fresh water. Next, the operator mixed and pumped 20 barrels of gel and 5 barrels of fresh water spacing followed by 125 sacks of Type 1 cement. The cement was displaced to 69 with 8 barrels of fresh water. I witnessed 9 barrels of good cement returned to surface. The cement held static at surface after 20 minutes.
1551453206 2019-03-01 UrbanProduction Wells No Violations I conducted an compliance notice follow-up inspection for compliance notice 243331845, regarding multiple compliance issues related to urban rule requirements. At the time of this inspection, identification still needed to be posted at the well. There was an identification sign posted at the tank battery, but was still missing the required information per 1501:9-9-05E(1)(a-e). The lease road gates were not locked, at the time of this inspection and the identification still has not been installed on the gate.

PAC needs to: 1) Post an identification sign to be in compliance with 1501:9-9-05E(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Post the proper identification sign on the access road gate per 1501:9-9-05E(1)(f). 3) Post an identification sign at the well per 1501:9-9-05E(2). 4) Install the locks on both access gates.

1693452334 2014-02-13 Completion Testing No Violations My inspection found the pits closed. PAC Drilling used 75 tons of Kiln Dust supplied by Chemtech. I noticed no violations at this time.
